If, during an internal structural, cargo tank internal examination, or underwater survey, damage or deterioration to the hull plating, structural members, or cargo tanks is discovered, the Officer in Charge, Marine Inspection, may require the vessel to be drydocked or otherwise taken out of service to further assess the extent of the damage and to effect permanent repairs. Make repairs called out in ABS/USCG worklist.

Electric installations on vessels must ensure Protection of passengers, crew, other persons, and the vessel from electrical hazards. MI's identified holes in the back of the High Voltage transfer cabinet cover plates that need repair/replacement within 30 days or during upcoming dry dock whichever is sooner.

Components are listed by a nationally recognized testing laboratory (NRTL) as that term is defined in 46 CFR 161.002-2, and are designed, installed, tested, and maintained in accordance with an appropriate industry standard and the manufacturer's specific guidance. Smoke detector in the galley was missing the required battery. New battery installed.

Fire pumps may be used for other purposes provided at least one of the required pumps is kept available for use on the fire system at all times. The fire pump designated as “available for use on the fire system at all times” was inoperable. Prior to departure, repair or replace to the satisfaction of the attending USCG Marine Inspector.

Each liferaft or group of liferafts must be arranged for float-free launching. The arrangement must ensure that the liferaft or liferafts, when released and inflated, are not dragged under by the sinking vessel. PORT and STBD (starboard) liferafts were permanently secured to the vessel. Repair or replace to the satisfaction of the attending USCG Marine Inspector.

The installation of all systems of an electrical engineering…nature…shall be in accordance with the details of design, construction, and installation of subchapter J (Electrical Engineering). Each generator arranged for parallel operation must have reverse-power or reverse-current trips. Both generators dropped offline and shutdown during testing of reverse power. Repair or replace to the satisfaction of the attending USCG Marine Inspector.

The installation of all systems of an electrical engineering or interior communication nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter J (Electrical Engineering) of this chapter. Each lighting fixture must be on a lighting branch circuit. External light on the Emergency Generator Room has a spliced lighting cable spliced into a 120V Receptacle. Repair or replace to the satisfaction of USCG Marine Inspector.

The installation of all systems of an electrical engineering or interior communication nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter J (Electrical Engineering) of this chapter. Emergency Generator Room has electrical penetration through bulkhead with improper RoxTec installation. Repair or replace to the satisfaction of USCG Marine Inspector.

The inspection shall be such as to ensure that the workmanship of all parts of the vessel and its equipment is in all respects satisfactory… Emergency escape hatch leading to the Lower Pump Room cannot be secured properly. Repair or replace to the satisfaction of USCG Marine Inspector.

At each inspection for certification and periodic inspection, the inspector shall examine the vessel to determine that it meets the vessel design and equipment requirements for pollution prevention in 33 CFR part 155, subpart B. Standard Discharge Connection is installed on wrong fitting and is corroded / wasted. Repair or replace to the satisfaction of USCG Marine Inspector.

No repairs or alterations affecting the safety of the vessel with regards to the hull, machinery or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspections. Vessel is dry dock and has work list items to complete.

(a) Except as provided in paragraphs (b) through (g) of this section, each vessel must undergo drydock, internal structural, and cargo tank internal examinations as follows: (1) Except under paragraph (a)(2) of this section, vessels that operate in salt water must be examined in accordance with the intervals set forth in Table 91.40-3(a) of this section. Where Table 91.40-3(a) indicates a 2.5 year examination interval, it means a vessel must undergo two examinations within any five year period. No more than three years may elapse between any two examinations. Subject vessel is past due drydock extension date of 29DEC22. Vessel is authorized for one time transit from Gulf Copper in Galveston, TX to Bollinger Ship Yard in Morgan City, LA. VSL is prohibited from commercial activity until dry dock inspection is completed. ***Action 15.c is articulated to mean to rectify deficiency upon arrival to Morgan City, LA.

Each fuel oil tank with at least one side integral to the vessel's hull and located within the hull (“integral fuel oil tank”) is subject to inspection as provided in this section. Fuel tank had a small pinhole, allowing fuel to discharge into bilge. Reinforce temporary repairs and make permanent repairs to the satisfaction of the attending USCG Marine Inspector.

In General, compliance with the standards established by the American Bureau of Shipping will be considered as satisfactory efficiency of the vessel. Vessel made a temporary repair of a 5" x 1" with a cement box 24” length x 12” width x 4” high made out of ½” thickness steel plate and welded to the bottom shell. During the temporary repair inspector noted significant wastage up to 40% via UT scan around the damage. Repair to the satisfaction of the Coast Guard Marine Inspection at next available dry dock. 46 CFR 92.01-10

In General, compliance with the standards established by the American Bureau of Shipping will be considered as satisfactory evidence of the structural efficiency of the vessel. Vessel reported a 5" x 1" hole in the bottom of the stbd storage area, a temporary repair of epoxy and wood has been applied. Repair to the satisfaction of the USCG Marine Inspector. Vessel is restricted from movement until deficiency is rectified.

In the event of an accident to a boiler, unfired pressure vessel, or machinery tending to render the further use of the item unsafe until repairs are made, or if by ordinary wear such items become unsafe, a report shall be made, by the by the Chief Engineer immediately to the Officer in Charge, Marine Inspection. Received report that the vessel's ladder winch system failed resulting in a gear box oil leak and pollution incident. Repair or replace system to the satisfaction of an attending Marine Inspector prior to conducting dredge operations.

The boundary bulkheads and decks separating the accommodations and control stations from cargo and machinery spaces, galleys, main pantries and storerooms, other than small service lockers, shall be of “A” Class construction. The fire door between the E/R and ECR does not properly close.

The E/R cable penetrations behind the paneling in the main stairwell does not stop the spread of fire / smoke.

Each launching station, or each two adjacent launching stations, must have an embarkation ladder as follows. The fender / liferaft embarkation ladder on the port side is being replaced due to damage and is not on board.

Box receptacle should be provided with covers. Receptacle outlet box facing the diesel transfer station is missing a cover.

Each fire pump must be capable of delivering water simultaneously from the two highest outlets at a Pitot tube pressure of approximately 50 p.s.i. The emergency fire pump was inop due to loose battery terminal connections.

General emergency alarm signal must be located in passenger and crew quarters areas where they can alert persons in spaces where those persons may be maintaining, repairing, or operating equipment, stowing or drawing stores or equipment, or transiting. The general alarm was inop in main stairwell and ECR

The emergency power source must meet table 112.05-5(a) and have the capacity to supply all loads that are simultaneously connected to it, except a load on a bus-tie to the main switchboard or non-required loads that are connected in accordance with §112.05-1(c). Ship’s EDG is currently inoperable.

Each pump must be capable of delivering water simultaneously from the two highest outlets at a Pitot tube pressure of approximately 50 p.s.i.. There are two seals leaking excessively resulting in inadequate pressure.

Each fireman's outfit must consist of one self-contained breathing apparatus, one lifeline with a belt or a suitable harness, one flashlight, one flame safety lamp, one rigid helmet, boots and gloves, protective clothing, and one fire axe. Both Fireman’s Outfits are missing tending lines and fire axes.

There shall be at least two means of escape from all general areas where the crew may be quartered or normally employed. At least one of these two means of escape shall be independent of watertight doors. The emergency escape hatch in the pump room overhead is not able to be opened (seized), resulting in only one means of escape from the space.

It shall be the duty of the owner, master, or person in charge to see that the vessel's firefighting equipment is at all times ready for use and that all such equipment required by the regulations in this Subchapter is provided, maintained, and replaced as indicated. The fire hose nozzles bails are seized and prevent the use of the fire hose.

General alarm light and bell are inoperable in the pump room.

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. There is excessive oil accumulation under the port/starboard SSDGs, oil soaked rags in bilges, HPU winch is leaking oil, HPU Aux #3 is leaking oil, diesel fuel is in improper containers, and there is excess fuel in the Racor fuel filter bowls, resulting in an extreme fire hazard.

The master of the vessel for which a load line certificate has been issued shall be responsible for the maintenance of such certificate on board such vessel and for compliance with its terms and conditions. #1 Port/STBD Void, #2 Port/STBD void, #3 Port/STBD void, and void aft of generators are all missing hardware, making them not in compliance with the load line and allowing for potential downflooding.

Ceilings, linings, and insulation, including pipe and duct laggings, shall be of approved incombustible materials. Wiring and cable runs do not meet fire protection standards for bulkhead penetrations.

At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. Three out of four self-igniting lights on lifebuoys are inoperable.

At each inspection for certification and periodic inspection, the inspector will check fire-extinguishing equipment with the following tests and inspections: (1) Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. Spare portable fire extinguishers are not stowed in proper locations or in proper brackets.

Electric installations on vessels must ensure Protection of passengers, crew, other persons, and the vessel from electrical hazards. Light fixture cover is missing in pump room, distribution panel is missing face plate, dead end wire behind air receivers, and open wiring enclosures near Port MDE and #3 Auxiliary HPO.

Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he deems necessary to be assured of the safety and seaworthiness of the vessel. Clutter in main engine room starboard side, makes eyewash and shower station inaccessible.

Pipe, tubing, pipe joining fittings, and piping system components, shall meet material and standard requirements of subpart 56.60. Numerous valve handles on Raw Water piping are missing.

The master or person in charge of a ship to which this section applies shall ensure that a written record is maintained on the ship of each of the following garbage discharge or disposal operations: (1) Discharge to a reception facility or to another ship; (2) Incineration on the ship; (3) Discharge into the sea; and/or (4) Accidental or other exceptional discharges. No entries have been made in the Garbage Record Book.

Remove old piping meant for OWS system and permanently cap end of piping.

Weld or permanently secure brackets holding verticle deck drainage piping in place.

Install missing electrical panel door and blanks to electrical panel in the spud room and electrical boxes in the electrical room adjacent to the galley.

Replace broken window in the spud room.

Fix hydraulic leaks to fittings and equipment in the spud room.

Provide properly working impulse line throwing apparatus.

Paint deck reference line of contrasting color.

Provide blank in electrical box EP1 in electrical room in galley.

Properly adjust fire door leading into galley from the outside. Door does not latch by itself.

Stbd side #1&2, Port Aft, & fwd mid blower vents missing several bolts. Provide missing bolt/nuts.

STBD #2 blower vent missing securing bolt for closure lid.

Properly clean galley vent damper.

Lube oil level gauge missing guard and shall be made of fire resistant material. Located in E-Gen room.

Properly secure PFD locker located IVO STBD side muster area.

Repair leaking shaft seal, suction valve, discharge pipe bracket and minor exhaust leak IWO fire pump.

Make scuttle in C6 watertight.

Properly secure all loose gear throughout the vessel to include hydraulic tank.

Provide parameters and test all shutdown/alarms for engines and ensure they are within parameters.

Provide proper fusible link IWO galley hood. Should be 212F and not 360F.

Remove excess oil from the fwd hydraulic room

Clean excess oil from bilge.

Repair all leaking unions and leaks on gear oil piping for the main pump engines and thrust bearing.

Provide an inspection certificate for the range hood fire extinguishing system prior to using deep fat fryer.

Install cover for junction box on stbd engine and port engine.

Install fire sleeves on all fuel hoses.

Remove exposed cable under STBD Gen.

Replace chaffed hose on the STBD pump engine.

Repair leaks on on the following pumps, Emergency fire pump, #1 and #2 SW cooling pumps and ladder pumps.

Replace gaskets on all escape hatches and WTH in all pump rooms.

Repair navigation light panel alarm and all navigation lights.

Install correct bracket for fire extinguisher on bridge.

Install insulation where missing in galley electrical room next to the GA box.

Replace trash can in galley with a fire retardant one.

Replace the aft fire hoses with approved fire hoses.

Secure lifejacket storage box.

Repair grease hose on main spud RAM stbd side.

Replace stbd life ring and repair light on the prt fwd life ring.

Port side containment needs to be enlarged to ensure full containment.

Issued 835 extending liferaft servicing date for 5 months or until next scheduled vessel exam whichever comes first as per 46 CFR 199.190(g).
