Electrical installations in hazardous locations must comply with paragraphs (b)(1), (2), or (3) of this section;NFPA 70 Articles 500 through 504 (incorporated by reference, see § 110.10-1 of this chapter). A permanent portable remote shutdown tether was installed that is not IAW NFPA 70 electrical installation requirements for hazardous locations, remove tether and return existing switch to explosion proof requirements. 46 CFR 111.108-3 NFPA 70/Article 505

The OCMI must be notified before alterations or modifications that deviate from approved plans. A new electrical connect for a portable cargo pump shutdown is being installed adjacent to the fixed cargo pump shutdown located within hazardous zone. Submit list of materials being used for modification. 46 CFR 110.30-7

Except as allowed in 33 CFR 151.33(a), no ship may carry a cargo of bulk liquid hazardous material or an NLS residue if the bulk liquid hazardous material or NLS is listed in Table 1 or carried under a written permission under paragraph (d) of this section unless the ship meets the following: The ship must have an IMO Certificate of Fitness issued under § 153.12 that is endorsed to allow the cargo tank to carry the cargo if it is A United States non-self-propelled ship in the waters of another Administration signatory to MARPOL 73/78 and the cargo is a Category A, B, or C NLS. The Cargo Authority Attachment (CAA) has NLS cargoes listed in IBC code, Chapter 17 & 18, which require the CAA to be updated and Certificate of fitness to carry these cargoes internationally. Vessel is prohibited from carrying NLS cargoes internationally until the CAA is updated by MSC and the vessel has been issued a Certificate of Fitness. Either submit plans through MSC for CAA verification of NLS cargoes for IBC code requirements or submit notification to our office that NLS cargoes will not be carried internationally. 46 CFR 153.900 (a)(4)

Stowage of flammable liquids and gases must be in containers approved by DOT or A.S.M.E for the contents carried. Gasoline cans, aerosols, and other flammable products are currently improperly stored in an auxillary equipment container creating a fire hazard. Remove all flammable products from the space.

Excessive oil has accumulated on the cargo pump engines and #2 generator creating a fire hazard. Clean engines and radiators of oil and repair the cause of the oil leaks.

Vessel must annually conduct a static liquid test on oil transfer hoses to 1.5 times the MAWP, verify cargo gauges are accurate within 10% of the actual pressure, and verify cargo pump relief valve relief at or below the rated pressure. Provide documentation showing these tests have been completed

An approved hazardous area plan showing all items listed in 46 CFR 111.105-31 (l) shall be provided. The current hazardous area plan does not include modifications to the general arrangement, additional electrical equipment added, and the addition of a hydraulic power unit space on top of the #1 Starboard Cargo tank. Provide plans meeting the criteria listed in MCS Plan Review Guide E2-12 to the CG Marine Safety Center for approval. To be completed no later than January 31, 2025.

No alterations involving the safety of a tank vessel either win regard to hull or machinery shall be made without the approval of the Commandant. Provide approved plans for the modification of a 20 foot container over the stbd #1 cargo tank into a hydraulic power unit space with associated electrical cable runs, lighting distribution panel and exterior lighting. If no approved plans are available then the required plans, encompassing all modifications, must be submitted to CG Marine Safety Center for approval. To be completed no later than January 31, 2025

Your vessel must undergo an annual inspection within 3 months before or after each anniversary date. The annual inspection was not completed within 3 months of the vessel's anniversary date, 01DEC2023. Complete annual inspection. 46 CFR 31.10-17 (a)

Vessels subject to initial and periodic surveys shall have annual surveys, within 3 months either way of the certificate's anniversary date. The annual surveys shall be made by and prove satisfactory to the assigning and issuing authority prior to executing the required annual endorsements on load line certificates or exemption certificates. The 3rd annual load line survey was not completed within 3 months of the certificate anniversary date, December 7, 2023. Complete required annual load line survey and provide copy of load line certificate with annual survey endorsement. 46 CFR 42.09-15 (d)

Install guards over rotating belts on both generators

fix broken turn buckle on port side life line and straighten bent stanson

Valves on deck were not all equipped with an indicator to show whether the valve was opened or closed. Ensure means to indicate if valve is open or closed. 46 CFR 38.10, 46 CFR 56.20-9

Expired SOPEP approval letter from CVC onboard. Provide current approval letter for plan. 33 CFR 151.27

Switchboard in prime mover space had unprotected openings. Provide means to protect crew from electrical hazards. 46 CFR 111.01-1 (2)

Vapor connection flange face for both port and staboard vapor lines did not have a permanent stud. Install permanent stud in accordance with 46 CFR 39.2001(j).

Load line marking no longer visible on starboard side. Ensure marks are plainly visible in accordance with 46 CFR 42.13-40.

Owner failed to conduct annual inspection in 2019 as required by 46 CFR 31.10-17(a).

Owner operated DBL 289 without a valid COI from April 14th 2019 to February 12th, 2020.

Owner operated DBL 289 without a valid IOPP certificate from April 14, 2019 to February 12, 2020.

Replace in kind anchor and chain.

Prove proper operation of at least one cargo pump prior to cargo ops following repairs.

Barge restricted from using remaining cargo pumps to transfer until repairs are approved by a Coast Guard Marine Inspector.

Cargo pump drive shaft seals left large gap. Replace with seals that would provide gastight bulkhead. 46 CFR 32.60-20(a)

Insert hull plating at Fr. 3-7.7 IWO Bottom Longs. 10-11, Fr.s 3-5 IWO Bottom Longs. 5-7, and Fr.s 5-7.5 IWO Bottom Longs. 4-5.

Insert bulkhead Fr #6 from Fr.s 3-4, and transvers Fr.s 4, 5, and 6 at Bottom Longs. 10-11. Insert Bottom Long #10 at Fr.s 3-4.

Make temporary repairs until dry-dock (Dec 2105). Gouge and weld broken welds to tripped longitudinal bulkhead 6 between frame 1 1/2 - 2 1/2 prior to departing the Port of Bethel. 46 CFR32.70-5

Make permanent repairs to forepeak hull plate and structural damage to include longitudinal bulkhead, swash bulkhead, transverse and longitudinal frames to the satisfaction of the attending marine inspector and class surveyor prior to receiving credit dry-dock. 46CFR32.70-5

Vessel shall not conduct cargo operations after lightering. Vessel shall have satisfactory repairs completed to the #1 void and verified by the attending Marine Inspector.

Provide approved plans for the anchor windlass hydraulic power unit (HPU), including hazardous location classification.

Installation & workmanship of the fwd anchor windlass must match approved plans to the satisfaction of the attending Marine Inspector. Operation is prohibited until the requirement is cleared.

Prove proper operation of the overfill alarm horn.

Crop and renew inset # 2 web frame IWO # 1 to # 3 side longs in # 4 void starboard side. Provide repair proposal for USCG approval prior to starting repairs. Requirement is extended to coincide with next hull exam due date of 31 December 2010.

Clean up indicated areas of possible leaks IWO suction box for # 1 port cargo tank. NDT test three suspect areas to the satisfaction to the attending USCG marine inspector. This item must be completed prior to carrying cargo in # 1 port cargo tank.

Properly weld reinforcing plates / brackets IWO starboard hatch in # 3 void. Provide repair proposal for USCG approval prior to starting repairs. Requirement was extended to coincide with the next hull exam due date of 31 December 2010.

Make all cargo high level alarms operational.

Provide cover for missing breaker in distribution oanel.

Remove all paints and hazardous combustibles from shed fwd deck area of barge.

Complete all repairs to port side hull where damaged by earlier event as per approved repair proposal.
