Vessel must be operated in such a manner as to afford protection against hazards. Vessel equipped with fire main to meet 46 CFR 142.325. Upon testing fire main an open valve in a branch line discharged a constant stream of water on the deck in the vicinity of the hose station. Valve could not be immediately closed due to missing handle from valve. A flooded deck without means to secure discharge would induce a hazard in an emergency. Temporary means employed (i.e. valve closed with add of tool), however, current configuration of branch line necessitates a permeant solution to eliminate potential hazard.

A remotely operated valve must be installed in the fuel piping directly outside of the fuel oil supply tank to stop the flow of fuel in the event of a fire or a fuel line break. At time of inspection remotely operated valves were located in branch lines in the vicinity of the engines being supplied and not directly outside of fuel oil supply tank (valves were installed at the proper location; however, they were not capable of remote operation).
