E/R Alarm in Engineer room found with disconnected and exposed wires on the obsolete engine room alarm system. Remove. 5. Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. This may be accomplished by an installed detection system, or by using individual battery-operated detectors meeting UL 217 (incorporated by reference, see § 136.112 of this subchapter). Detection systems or individual detectors must be kept operational at all times when the crew is onboard the towing vessel. Three (3) individual independent smoke detectors found in staterooms to be not operational. Replace smoke detectors. - 46 CFR 142.330(b) - 10c

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. Watertight collision bulkhead is missing to have welded and installed a watertight hatch cover. Complete the welding and installation of the collision bulkhead watertight hatch.

46 CFR 143.400 (a) Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Port and starboard generator windings are coated in excess oil and dirt, and require cleaning.

46 CFR 140.715(a) Towing vessels must meet the communications requirements of 33 CFR part 26 and 33 CFR 164.72, as applicable. Vessel did not have a Bridge-to-Bridge Safety Certificate.

46 CFR 144.320 (b) Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. Engine room port lights are missing dead covers.

46 CFR 140.610(b) The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. Pilot house hatch striker plate damaged due to a temp repair or hatch dog.

All towing vessels must be operated in a manner that minimizes the risk of down-flooding and progressive flooding. Stbd engine room door leading to engine room was missing striker plate. Repairs made and chalk tested.

Received email from vessel representative that the starboard generator is inoperable and a portable generator was installed without notice to the OCMI.

At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light approved under approval series 161.010 or 161.110, unless the towing vessel is limited to daytime operation, in which case no floating electric water light is required. MI found spring terminal battery instead of screw post battery terminal, not properly working as designed. Provide proper battery to the satisfaction of attending MI. 46 CFR 141.360 (c)(2)

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. The WTD on the port side was found with a section of missing gasket near the top knife edge and needs replacing. 46 CFR 140.610

Install a break-glass cover on Kidde pull-box in galley.

Prior to conducting operations with fire supression system ventilation shutdown system must be tested and witnessed by attending Coast Guard Marine Inspector.
