The vessel operator of each vessel required by § 155.720 to have transfer procedures shall maintain them current and shall require vessel personnel to use the transfer procedures for each transfer operation. The vessel's transfer procedures require a preventer pin to be installed top dead center of each flange bolt circle, designed to further identify these flanges as vapor recovery connections and not for the transfer of liquid cargo or ballast. Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 millimeter (0.5 inch) diameter and is at least 25.4 millimeters (1 inch) long. It must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern. The port and starboard vapor recovery piping located at the manifold does not have a stud installed top dead center on the fixed piping flange face.

Company Form FL 03/21 states passwords shall be strictly controlled. Removing visible passwords, sticky notes on monitors, under key boards, or inside draws. MI observed 03 computers displaying log-on and passwords.
Coast Guard requires an internal audit of the vessel's Cyber Security Plan within 90 days.
