The endorsements shall each be assigned a unique number, matching the certificate concerned. PSCO observed incorrect number on endorsement. STCW I/2.8.3 (16 a,c)

Sanitary facilities for all members of the crew who do no occupy rooms to which private or semi-private facilities attached shall be provided one water closet for every 8 persons or less. PSCO observed deck one centerline water closet inoperable. PSCO observed starboard water closet with unapproved water supply. ILO 147 crew accommodation/ para.20 PWSA46 USC 70002

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. PSCO observed port lube oil pump leaking. 74SOLAS(02) II-1/26.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and any additional requirements which may be established by the company. In meeting these requirements, the company should ensure that: inspections are held at appropriate intervals, any non-conformity is reported, with its possible cause, if known; appropriate corrective action is taken, and records of these activities are maintained. The technical and operational nature of the deficiencies noted below provide objective evidence of a serious failure of the implementation of the ISM Code. An external audit is required prior to clearance of this deficiency. 74SOLAS(20) IX/3.1, ISM Code 10.1 & 10.2 - 30bc

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects remains fit to proceed to sea without danger to the ship or persons onboard. PSCO observed active hydraulic oil leak from the port side piston ram on steering gear while conducting a steering test. 74 SOLAS (20) I/11(a) - 30bc

All gearing of every shaft and coupling used for transmission of power to machinery essential for the propulsion and safety of the ship… shall be so designed and constructed that they will withstand the maximum working stresses to which they may be subjected in all service conditions. Main engine stern tube leaking oil at a rate of 1 drop every 10 seconds. 74SOLAS II-1/27.3 - 30bc

Oil fuel pipes and their valves and fittings shall be of steel or other approved material. To control the fuel, lubrication, and hydraulic oils leaking from machinery, the crew cut out holes in metal plating, manufactured fittings, attached flexible hoses, buckets, and funnels to divert and collect leaking oil. Where the contents and use of a space are such that there is doubt as to its classification for the purpose of this regulation, or where it is possible to assign two or more classifications to a space, it shall be treated as a space within the relevant category having the most stringent boundary requirements. Multiple void spaces are being left open and entered by ship's crew for use of storage of paint, chemicals, spare parts, garbage and other items. Cite: 74 SOLAS (04) II-2/9.2.3.3.2.2 Action: 17bc

After any survey of the ship...has been completed, no change shall be made in the structural arrangements, machinery equipment and other items covered by the survey without the sanction of the Administration. If a ship at the time when a certificate expires is not on a port in which it is to be surveyed, the Administration may extend the period of validity of the certificate but this extension shall be granted only for the purpose of allowing the ship to complete its voyage to the port in which it is to be surveyed, and then only in cases where it appears proper and reasonable to do so. Vessel was last in drydock October 2018/ two subsequent short term SOLAS Cargo Ship Safety Construction Certificates were issued from 06OCT23-26JAN24 and 01JAN24-26APR24 for the purpose of extending the drydock date. FIRE-FIGHTING SYSTEMS AND APPLIANCES SHALL BE KEPT IN GOOD WORKING ORDER AND READY FOR IMMEDIATE USE. CO2 FLEX HOSE CONNECTIONS WERE FOUND LOOSE.

Maintain a ballast water management plan that has been developed specifically for the vessel. Vessel used white out to correct multiple wrong entries. Corrected on the spot.

The ring lines and letters shall be painted white or yellow on dark background. Plimsoll mark is rusted over with no paint.

Before the ship leaves port and at all time during the voyage, all life-saving appliances shall be in working order and reday for immediate use. Rescue boat engine failed to start.

The fire control plan shall be kept up to date; any alterations thereto shall be recorded as soon as practicable. Port State Control Examiner identified that the galley door is missing a self-closing device preventing it from operating as a self-closing door. Per the fire control plan, the galley door shall be a self-closing door.

A jacket water leak was identified on the #4 main engine cylinder. This equipment is not functioning as designed and presents a danger to the ship and the crew.

The fire safety objectives of SOLAS II-2 are to prevent the occurrence of fire and explosion and reduce the risk to life caused by fire. Found No. 1 service generator leaking fuel at approximately 1 drop every 4 to 5 seconds in the vicinity of No. 6 cylinder head, and onto the hot engine block. 04 SOLAS Ch II-2 Reg 2.1

0750 - Fire Prevention - Doors fitted in boundary bulkheads of machinery spaces of category A shall be reasonably gastight. Reefer machinery space was found with a temporary sliced electrical cable operating a 22V electrical fan for the vegetable reefer stores. The temporary cable was running through the A-15 Reefer Machinery boundary door... SOLAS 74 (2004) Ch. II-2 Reg. 9.4.2.1

0720: Machinery space containing oil fired boiler does not have portable extinguishers or sand box meeting the requirements outlined in II-2 Reg 10.5. Approved fire control plan does not show these items as being required in this space and no exemption has been issued to the vessel. Administration to attest to proper installations. 0799: Numerous self closing "A" class doors will not fully close. Most notable doors listed: Door leading to boiler, door outside of funnel across from boiler, both doors (lower & upper engine room) leading to the emergency escape trunk, and door leading to the steering flats. SOLAS 74 as amended Ch. II-2 Reg 9.4.2.1. 16(a)

0960: Door frame to emergency escape trunk located in steering flats has a visible gap between the bulkhead and frame. Light visibly passes through at least two screw holes used to attach frame to bulk head. SOLAS 74 as amended Ch.

C/E is not maintaining/logging entries in the Oil Record Book as described in Appendix III of MARPOL. Training should be conducted as to proper implementation of ORB keeping. Weekly soundings also not being performed. MARPOL Annex I reg 17.

C/E is using milk for the testing of the 15 PPM equipment. Manufacturer's recomendations only mention the use of a brush for cleaning. C/E was unable to provide any information that his techniques were of an accepted/approved method. Class to provide report of proper calibration of 15PPM equipment.

Vessel does not have a vessel specific Ballast Water Management Plan as outlined in 33 CF 151.2035(a)(7). Rectify deficiency prior to next US port after sailing foreign.

Owner/operator failed to prepare and submit a nontank vessel response plan IAW 33 U.S.C 1321 (j)(5).
