Whenever an accident occurs to a ship or defects are discovered, either of which affects the safety of the ship or completeness of it's life-saving appliances or other equipment, the master or owner of the ship shall report at the earliest opportunity to the Administration, the nominated surveyor or Recognized organization responsible for issuing the relevant certificate, who shall cause investigation to determine whether a survey as required by regulation 7,8,9 or 10, is necessary. PSCO Received notification from Sector Command Center that vessel had a loss of propulsion while enroute to berth at Seagirt Marine Terminal, Baltimore, MD. 74 SOLAS (20) I/11c

PSCO observed unapproved modification on the boilers air blower piping. Additionally, the boiler burner door does not secure properly, leaving a gap on top of the door. Fuel accumulation was observed. SOLAS (2004) II-1/26.1

A review of the vessel's PMS status indicated 13% total PMS items overdue and 19% of critical PMS overdue. An external ISM audit is recommended. 74 SOLAS 14 IX/3 ISM Code 10.2
