Whenever an accident occurs to a ship or defects are discovered, either of which affects the safety of the ship or completeness of it's life-saving appliances or other equipment, the master or owner of the ship shall report at the earliest opportunity to the Administration, the nominated surveyor or Recognized organization responsible for issuing the relevant certificate, who shall cause investigation to determine whether a survey as required by regulation 7,8,9 or 10, is necessary. PSCO Received notification from Sector Command Center that vessel had a loss of propulsion while enroute to berth at Seagirt Marine Terminal, Baltimore, MD. 74 SOLAS (20) I/11c

The OWS system is designed to drain from the containment, through the piping arrangement, into the bildge holding tank. the piping arrangement was blocked causing an overflow from the OWS containment.

Means shall be provided to control leaks of flammable liquids. PSCO observed fuel oil accumulation around the injector fuel line connection point on #1, #8, and #9 cylinder heads. There was hydraulic oil leak on #7 hydraulic return line and accumulation around #9 cylinder head. Addionally the incinerator fuel sample line was leaking. SOLAS (2020) II-2/4/1/1

The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be installed and protected as to reduce to a minimum any danger to persons onboard. PSCO observed unapproved modification on the boilers air blower piping. Additionally, the boiler burner door does not secure properly, leaving a gap on top of the door. Fuel accumulation was observed. SOLAS (2004) II-1/26.1

PSCO observed jacket water leaks around #5 and #12 ME cylinder heads. SOLAS (2004) II-1/26.1

All electrical apparatus shall be constructed and so installed as not to cause injury when handled or touched. PSCO observed exposed wiring on all oil purifiers. SOLAS (2004) II-1/45.1.3

Any non-conformity is reported, with its possible cause, if known; appropriate corrective action is taken. Condition of machinery in engine room was found lacking in maintenance as indicated by oil leakage and multiple catch-buckets in the space. A review of the vessel's PMS status indicated 13% total PMS items overdue and 19% of critical PMS overdue. An external ISM audit is recommended. 74 SOLAS 14 IX/3 ISM Code 10.2

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Drain lines for high pressure fuel pumps were found disconnected on multiple cylinders of the main engine. 74 SOLAS 14 I/11(a)

Means shall be provided to control leaks of flammable liquids. 05 oil catch buckets located in the engine room were found full of oil. 74 SOLAS 14 II-2/4.1.1

Means shall be provided to control leaks of flammable liquids. Fitting for waste oil drain line was found disconnected, leaking, and wrapped in rags. 74 SOLAS 14 II-2/4.1.1

Every steam pipe and every fitting connected thereto through which steam may pass shall be so designed, constructed and installed as to withstand the maximum working stresses to which it may be subjected. Valve on low sulphur fuel oil number 7 starboard heating steam line was found leaking. 74 SOLAS 09 II-1/33.1

0730 - Fire Prevention - Doors required to be self-closing shall not be fitted with hold-back hooks. There were 05 doors required to be self-closing on the B-deck that were fitted with hold-back hooks. SOLAS74(04Cons.)II-2/9.4.2.2

Upon deck walk inspection, discovered forward mooring lines on winch M2 and winch W2 frayed beyond blue indication wear lines. Replacement lines on board. Replace worn lines by next port to the satisfaction of the Coast Guard per SOLAS Ch. 1 Reg. 19 a.

The S-Band Radar onboard the vessel is inoperable, leaving the X-Band Radar as the only operable radar onboard. The inoperable S-Band Radar must be repaired before the vessel departs San Francisco Bay. A facsimile documenting satisfactory repairs from a certified technician will be accepted.
