Machinery must be designed and maintained to provide safe operation of the towing vessel under normal and emergency conditions. Port Main engine received a low oil pressure alarm necessitating the engine being secured, reducing the vessel's propulsion capability for towing operations. Repair port propulsion unit to the satisfaction of TPO prior to towing operations. In accordance with CVC Policy Letter 17-10, the above deficiencies may be cleared by the operating company's TPO (ABS). TPO must submit a report to the cognizant OCMI within 72 hours of clearing the deficiencies.

The owner and managing operator must ensure that the towing vessel is in compliance with this subchapter and other applicable laws and regulations at all times. The starboard side hull suffered damage to the bottom plating 25' back from the bow. The damage consisted of a dent 2'x3' with a 1/4 wide fracture that is 6" to 8" long resulting in water ingress to the common void below fuel tanks. The vessel is authorized to proceed form the Wepfer Marine fleet to James Marine in Wicckliffe, KY to complete permanent repairs. Transit must be conducted in accordance with the vessel's transit plan approved by the United States Coast Guard Sector Lower Mississippi River. Contact the Coast Guard as soon it is safe to do so if the situation on board the vessel deteriorates or anything changes. All repairs must be to the satisfaction of the TPO (ABS). Also, ABS surveyor must provide Sector Lower Mississippi River objective evidence that all repairs are satisfactory prior to commercial operations.

Vessel was involved in a marine casualty - allision with barge. A dive survey of vessel reported the following damage: 1.Stbd interior flanking rudder is missing; 2.Stbd exterior flanking rudder is bent outward at breakaway point; 3.Stbd main rudder is broken off at breakaway point; 4.Stbd prop flukes all have small golf ball size bends on tips; 5.Port flanking rudders are both bent outward at breakaway point. Make repairs to the satisfaction of attending CG Marine Inspector prior to movement. The vessel has been authorized a one-trip transit in accordance with the submitted transit plan from MM 113 LRM to Bollinger Quick Repair Shipyard. 46 CFR 143.205(a) 60c
