13 - Propulsion and Auxiliary Machinery › N/A - No Subsystem › Gauges, thermometers, etc
Issued 24 January 2025
Resolved
The following systems must be equipped with gauges at the machinery location:
1) Main engine lubricating oil pressure and main engine RPM;
(2) Main engine cooling water temperature;
(3) Auxiliary generator engine lubricating oil pressure and auxiliary generator engine RPM;
(4) Auxiliary generator engine cooling water temperature; and
(5) Hydraulic steering fluid pressure, if the vessel is equipped with hydraulic steering systems.
Both generators are missing RPM gauges. Add RPM gauges for both generators. Ensure main engine RPM gauges are operating satisfactorily.
Condition: Improper Installation
Action required: 16 - Rectify deficiencies w/in 14 days
Due 10 February 2025
Resolved 26 February 2025
Resolution: New RPM gauges installed for generators. Both main engine RPM gauges repaired.
13 - Propulsion and Auxiliary Machinery › N/A - No Subsystem › Operation of machinery
Issued 23 February 2024
Resolved
Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must: (1) Be maintained to ensure proper operation; (2) Be suitable for route and service
Flanking rudders inoperable.
Troubleshoot and repair flanking rudders to regain full steering control.
Condition: Improper/Lack of Maintenance
Action required: 17 - Rectify deficiencies prior to departure
Resolved 24 February 2024
Resolution: Electrical technician found blown fuse is hydraulic pump motor controller. Fuse replaced; operational test of flanking rudders and hydraulic pump SAT.
03 - Water/Weathertight Conditions › N/A - No Subsystem › Machinery space openings
Issued 23 February 2024
Resolved
Decks and bulkheads designed to be watertight or weathertight must be maintained in that condition.
Bulkhead penetrations found in forward and aft machinery space.
Repair by welding penetrations shut or fill penetrations with a fire rated material.
Condition: Improper/Lack of Maintenance
Action required: 50 - Rectify deficiencies w/in 30 days
Due 23 March 2024
Resolved 3 April 2024
Resolution: An approved firestop epoxy was used to fill all bulkhead penetrations.
07 - Fire Safety › N/A - No Subsystem › Remote Means of control (opening,pumps,ventilation,etc.) Machinery spaces
Issued 23 February 2024
Resolved
To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover.
Large 30-50 gallon fuel pot found after the fuel shut off valve between all engines. Fuel pot prevents the flow of fuel from stopping in the event of a fire or break in the fuel line.
Remove the fuel pot or install additional emergency shut off valves after the fuel pot before each engine fuel supply line.
Condition: Design Flaw
Action required: 705 - Other - as specified
Due 22 April 2024
Resolved 3 April 2024
Resolution: New fuel shut off cable assembly tested satisfactory. Fuel pot remains on the vessel, but the fuel shut offs are after the fuel pot.
07 - Fire Safety › N/A - No Subsystem › Other (fire safety)
Issued 3 November 2022
Resolved
Fire hazards to be minimized. Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: (a) All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable.
Primary containment at port/stbd fuel stations contain an exposed pipe opening that would allow overflow to drain directly to engine room bilge. Pipe opening must be capped or otherwise secured to prevent overflow into engine room bilge.
Condition: Design Flaw
Action required: 60 - Rectify deficiencies prior to movement
Resolved 4 November 2022
Resolution: Plugged pipe hole in secondary containment with an appropriate sized rubber/stainless locking device designed for pipe fittings.
07 - Fire Safety › N/A - No Subsystem › Oil accumulation in engine room
Issued 3 November 2022
Resolved
Fire hazards to be minimized. Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: (a) All bilge and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable.
Excess oil accumulation in engine room. Remove excess oil accumulation.
Condition: Improper/Lack of Maintenance
Action required: 60 - Rectify deficiencies prior to movement
Resolved 4 November 2022
Resolution: Removed all excess oil contamination and wiped bilges clean.
08 - Alarms › N/A - No Subsystem › Machinery controls alarm
Issued 3 November 2022
Resolved
Alarms and monitoring. Each vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention.
Vessel does not have a means of conducting an operational test of the main engines lubricating oil pressure, cooling water temperature, auxiliary generator lube oil pressure, cooling water temperature, or low hydraulic steering fluid levels. Vessel shall provide reliable means to test these alarms IAW manufactures specifications and/or standard engineering practices for inspectors to witness.
Condition: Improper/Lack of Maintenance
Action required: 50 - Rectify deficiencies w/in 30 days
Due 22 February 2023
Resolved 6 February 2023
Resolution: Received alarm settings / test results. All Sat.
