Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. The port and starboard running lights were positioned in such a way that they were not visible to an approaching vessel. Properly reposition both port and starboard running lights and demonstrate proper operation of the anchor light to the satisfaction of an attending Coast Guard Marine Inspector prior to February 11, 2021.

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. The stern light was burning an amber color as opposed to a white light in accordance with 33 CFR 83.21(c). Remove the amber bulb and replace it with a white light to the satisfaction of an attending Coast Guard Marine Inspector prior to February 11, 2021.

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway in accordance with 46 CFR 140.635. The master was not conducting a navigational assessment that met all items as listed in the regulations. Safety orientation for non-crew members who are getting underway with the vessel must be completed and documented in the TVR in accordance with 46 CFR 140.415. The vessel did not have any records of a Safety Orientation for non-crew members who recently embarked on the vessel. Properly document the safety orientation for all non-crew members getting underway on the vessel in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to February 11, 2021.

Ships generator buss panel has 100 AMP listed as required breaker size for generators, Port and Stbd generator have 200 and 225 AMP breakers installed. Ensure breakers providing over-current protection for generator are properly sized to generator. Ensure wiring from generator to breaker is properly sized. Ensure Stbd generator is load tested after installation and prior to operations.

The operator of a vessel which is compulsory to the Bridge-to-Bridge Act must have an appropriate FCC Radio Operator Permit; the attending Captain does not possess the requisite FCC permit.

Each towing vessel, over 26 feet, must be equipped with a radiotelephone installation in accordance with the Bridge-to-Bridge Act; the operator is not able to provide proof of certification indicating the radio station is in compliance with the FCC regulations.
