Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. It was reported that a cooling water pipe for the main generators running through a fuel tank was damaged, causing contamination of fuel, and leading to a loss of power.

Resolution: Issued found to be untighten flange. Tightened flange and test ran, all sat.

A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. It was reported that the vessel sustained structural damage due to contact made with the barge in tow.

To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. Vessels stbd side fuel shutoff valve failed to operate.
