Electrical systems and equipment must function properly and minimize system failures and dire and shock hazards. a portable surge protector was observed being used as the primary power source for the submersible pump, chargers and equipment in use in the engine room. Provide power to the equipment using permanent means.46CFR143.400

Alarms must be visible and audible. The alarm at the operating station may be a summary alarm; if the alarm at the operating station is a summary alarm, the specific alarm condition must be indicated at the machinery or bilge location. The vessels bilge indicators in the machinery room had a shared alarm for the Laz/ER. 46CFR143.230

Switchboard was missing non conductive mat, Open gaps were observed where planking plates or protective covers should be fitted and cleaning supplies were found stored behind the switchboard.46CFR143.400

A 40B fire extinguisher must be located near a storage room or cabinet carrying flammable or combustible products. 2 storage rooms labeled as flammable lockers were found without a 40B extinguisher nearby. 46CFR142.225

Vessel did not have documentation of fire, abandon ship, man overboard or flooding drills. Drills must be kept in the TVR, or official logbook. 46CFR140.420

Vessels steering tower appeared to be inoperable, crew reported that it is no longer functional, however electrical hazards were found in the tower. Submit a determination to decommission the steering tower and move vital/electrical equipment to the wheel house or remove hazards and prove satisfactory operation of navigational equipment. 46CFR140.205

A complete application for an original MMC must contain proof, documented on CG-719-K or CG-719-K/E, as appropriate, that the applicant passed all applicable vision, hearing, medical, and/or physical exams as required by subpart C of this part or a valid medical certificate issued by the Coast Guard. The Master did not have his medical certificate available. Provide master who has valid medical certificate to meet manning requirements. 46 CFR 10.302(a)

Pressure vessels over 5 cubic feet in volume and over 15 pounds per square inch maximum allowable working pressure (MAWP) must be equipped with an indicating pressure gauge and with one or more spring-loaded relief valves. The total relieving capacity of such relief valves must prevent pressure from exceeding the MAWP, as established by the manufacturer, by more than 10 percent. Pressure vessels must be externally examined annually. Relief valves must be tested in accordance with § 143.245. All pressure vessels must have the MAWP indicated by a stamp, nameplate, or other means visible to the crew. MAWP stamp, nameplate not visible. Provide name plate for pressure vessel. 46 CFR 143.300

Essential systems or equipment must be regularly tested and examined. Tests and examinations must verify that the system or equipment functions as designed. Pressure vessel relief valves need to be tested twice every 5 years, with no more than 3 years since last test. Testing of pressure relief valves not completed. Complete testing of pressure relief valves. 46 CFR 143.245

The aft hydraulic steering ram was found with 3 of 4 bolts attaching it to the framing structure and one bolt was found to be lose. Provide hydraulic ram with 4 bolts and properly torque all hardware. Cite: 46 CFR 143.205

(a) Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. Vessel went into emergency dry dock for repairs due to unusual noise and vibrations from starboard engine and shaft.

When conducting an examination of a towing vessel as required by this subpart, the surveyor must determine whether any defect, deterioration, damage, or modifications of the hull and related structure and components may adversely affect the vessel's seaworthiness or fitness or suitability for its route or service. (b) The examination must address the items in § 137.330 as applicable, and must include: (1) Access to internal spaces as appropriate; (2) A visual examination of the external structure of the vessel to confirm that the condition is properly maintained; and (3) A visual examination to confirm that unapproved modifications were not made to the vessel. (c) The thoroughness and stringency of the examination will depend upon the condition of the vessel. (d) The owner or managing operator must notify the cognizant OCMI when the condition of the vessel may create an unsafe condition. The vessel is at Marisco Shipyard, Honolulu Hi, conducting repairs without Coast Guard Oversight. Vessel is a Coast Guard Option inspected vessel under 46 CFR 137. Vessel must complete CG inspection of repairs prior to any towing operations.

Engine room remote fuel shut off valves for both PORT & STBD generators failed to operate; faulty. Repair or replace valves.

Steering control electrical enclosure panel cover missing at Portside steering unit; re-install cover.

Heat detector in overhead of engine room found hanging from wiring; remount.

Vessel flammable lockers found to have paint spilled and oil soaked rags being stored inside. Clean spilled paint and discard oil soaked rags.

Flammable lockers found to have ventilation holes in the sides preventing CO2 extinguishers from being effective. Plug holes if flammable lockers.

Stair-tower providing access to top control station found to be missing emergency lighting. Install emergency lights.

Aft Towing station 02 weather deck lighting missing bulb diffuser. Re-Install bulb diffuser.

PORT side life ring buoy; remove permanent lanyard that secures marker light to buoy, renew ring buoy line and retro-tape.

Port side Deck Flammable locker fire extinguisher at station #12 servicing by GFS expired on November 2020.

Remove dryer ducting from galley exhaust hood and replace with rigid piping.

Repair STBD MDE exhaust lagging where holed. (near winch generator and in stack)

PORT F/O SHUTOFF VALVE IS INOPERABLE FROM OUTSIDE THE SPACE. ENSURE PROPER OPERATION PRIOR TO DEPARTURE.

PORT AFT F/O ISOLATION VALVE FOR TRANSFER PIPING IS INOPERABLE. ENSURE PROPER OPERATION AND DOCUMENT IN VESSEL FUEL SYSTEM MAINTENACE PLAN AS REQUIRED.

ENSURE ALL ESCAPE HATCHES ARE MARKED ON BOTH SIDES IN AT LEAST 2" LETTERS "EMERGENCY EXIT, KEEP CLEAR".

INDIVIDUAL CIRCUIT BREAKERS ON SWITCHBOARDS AND DISTRUBUTION PANELS MUST BE LABELED WITH A DESCRIPTION OF THE LOADS THEY SERVE.

INSTALL BLANKS ON ELECTRICAL PANEL TO MINIMIZE SHOCK HAZARDS TO PERSONNEL.

PROPERLY INSTALL A SAFETY GUARD OVER ROTATING MACHINERY.

IDENTIFY SOURCE OF HYDRAULIC LEAK IN AFT ENGINE ROOM CENTERLINE.

PROPER MOUNT F/O STAINER/FILTER HOUSING FOR PORT SSDG.

Vessel must provide required life raft prior to departure. Vessel had no liferaft at time of Inspection.

Vessel is restricted to Domestic Service until vessel meets SOLAS requirements

Bilge Alarm is required to be audible at operating station. Alarm was not audible.

Vessel's COD shows Hailing Port as New York, NY. Vessel must update COD to reflect San Diego, Ca. as the Hailing Port.

Failure to comply with requirements for oil transfer. Cite: 33CFR156.120

PROVIDE PROOF EPIRB IS REGISTERED WITH NOAA, AND AFFIX NOAA DECAL TO EPIRB.
