Steering systems must meet ABS Rules for Building and Classing Steel Vessels under 90 Meters (295 Feet) in Length (incorporated by reference, see § 136.112 of this subchapter), Part 4, Chapter 3, Section 3. Vessel's rudder post packing gland hardware and structural ring was severally corroded. Provide repair proposal. 46 CFR 143.550

Pressure vessel relief valves shall be tested Twice every 5 years, with no more than 3 years elapsing since last test. Vessel had two tags on the installed pressure relief valve indicating two different pressures, only one report could be produced to show the testing was done in 2023. Relief valves shall be tagged and tested with the correct pressure. 46 CFR 143.245(b)

Each towing vessel operating in oceans and coastwise service must have a line throwing appliance approved under approval series 160.040. Four rocket projectiles, each complete with bridle and leader of fire-resistant materials. Two of the projectiles shall be of the buoyant type. 46 CFR 141.385

a) Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. b) Alarms must: (1) Be visible and audible at each operating station. The alarm located at the operating station may be a summary alarm; if the alarm at the operating station is a summary alarm, the specific alarm condition must be indicated at the machinery or bilge location. MI observed no alarms, visible or audible, at either operating station. Vessel must repair alarm panel at each operating station (pilothouse and aft). If aft operating station is not to be used it must be secured and removed from service. 46CFR143.230

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel experienced loss of steering around LMR 138. Vessel will provide technicians report and statement from master attesting to what occurred as well as the proper repair, testing, and operation of the steering system prior to movement.

Individual circuit breakers on switchboards and distribution panels must be labeled with a description of the loads they serve. One unlabeled breaker (number 23) was present and 03 energized breakers were labeled "blank". Correctly label all breakers and secure power to any blank breakers.

Electrical components that pose an electrical hazard must be in an enclosure. Dead end wires and unused equipment were found in engine room and forward compartment. Remove dead end wires or secure in a junction box. Ensure power to unused equipment is secured if the equipment and cabling is not removed.

Piping for flammable or combustible liquid, seawater cooling, or firefighting systems must be fitted with isolation valves that are clearly marked by labeling or color coding that enables the crew to identify its function. One valve in engine room was not labeled. Identify and label valve. Dead end pipe was present in engine room near forward bulkhead. Identify pipe usage and label/cap correctly, or remove pipe.

Vessel must be provided with a installed fire suppression system for the vessel's engine room. Vessel's installed CO2 system had 02 corroded and worn hoses.

Vessel must have a fire detection system. Vessel's fire detection panel had a battery trouble light.

Vessel must have a bilge pump for dewatering. Vessel's bilge/ballast pump had a leak.

Pressure vessel's over 5 Cubic feet and over 15 pounds per square inch maximum allowable working pressure must be equipped with one or more spring-loaded relief valves and be inspected twice in five years with no more than 3 years elapsing since last test. Vessels forward air compressor tank has spring loaded relief valve set at 250psi, system's MAWP is 200psi, thus exceeding the 200psi overage by more than 10 percent. Vessel is able to operate with one tank and is able to isolate the other tank with the out of range valve. Vessel shall operate solely with the aft tank with the properly adjusted valve. Only in emergencies should the forward tank be used until the valve is adjusted or replaced with correctly adjusted valve PSI relief.

Vessel fire detection system was not serviced in the last 12 months, vessel is required to have system vessel serviced by a third party.

Line throwing device was missing the required 1500ft auxiliary line, vessel is required to provide required auxiliary line.

UPV located in engine room had a pressure relief valve that was not serviced since 2015. Vessel is required to either replace valve or have it serviced.

E/R general alarm lens missing an on order.

Device had no propellant canister so it could not be armed or used.

Generator windings has excessive dust build up. Windings could not bee seen.

Vsl has no means to recover a person from the water with the normal operating draft freeboard of 3' 9". This seems to be an oversight in M.
