Issued 17 October 2022
Resolved
Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. During loss of power to the vessel, the navigation lights were inoperable. Demonstrate adequate alternate energy source for the navigation lights during emergency operations following a loss of power to the vessel.
Action required: 16 - Rectify deficiencies w/in 14 days
Due 31 October 2022
Resolved 2 November 2022
Resolution: Witnessed proper operation of navigation lights.
09 - Working and Living Conditions › 092 - Working Conditions › Electrical
Issued 17 October 2022
Resolved
Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Marine Inspector found that port generator did not have a functioning amp meter, starboard generator did not have an amp meter, voltage meter, and frequency meter. Ensure proper operation of meters for both generators.
Action required: 16 - Rectify deficiencies w/in 14 days

Issued 17 October 2022
Resolved
Closure devices must be provided for deck-house or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. Vessel is fitted with a dripless shaft seal, Marine Inspectors found both shaft seals leaking. Installed shaft seals should be installed and function per manufacture specifications.

Resolved
Any piping system that penetrates the hull below the waterline must be fitted with an accessible valve, located as close to the hull penetration as practicable, for preventing the accidental admission of water into the vessel either through such pipes or in the event of fracture of such pipe. Marine Inspector found that Non-Integral keel cooler installations for the Port and Starboard Main Engines and Port and Starboard Generators have flexible hose installed between the hard piping and through hull fitting. The installation of a valve meeting the requirements noted is required to be installed.
Action required: 16 - Rectify deficiencies w/in 14 days
Due 17 November 2022
Resolved 29 November 2022
