At each inspection for certification and periodic inspection, the quarters, toilets, and washing spaces, galleys, serving pantries, lockers, etc., shall be examined by the inspector to be assured that they are in a sanitary condition. The frame or support of each semi-portable fire extinguisher required by Table 95.50–10(a) must be welded or otherwise permanently attached to a bulkhead or deck. Fire extinguisher locations #6 and #30 have fire extinguishers unmounted. 46CFR95.50-20(a)

Non-Metallic flexible hose may only be used in lube oil, fuel oil and fluid power systems only where flexibility is required and in lengths not more then 30 inches. Each fire pump must be capable of delivering water simultaneously from the two highest outlets at 50 psi. Vessels fire main pipes were found with pin holes. Replace/repair fire main piping and demonstrate proper operation. 43CFR95.10-5(c)

General construction of the vessel shall be such as to minimize fire hazards. Vessels generator fuel return line leaking at the check valve. Replace/repair valve. 46CFR92.05-1

Suitable hand covers, guards, or rails shall be installed in way of all exposed and dangerous places. Vessel missing louvre on vent fan. Replace louvre. 46CFR92.25-15

Enclosures for each motor controller must meet IEC 60092-201 standards. Tag line winch motor controller unable to lock and was not watertight. Replace/repair motor controller. 46CFR111.70-3(a)

Electric installations on vessels must ensure the protection of the crew from electrical hazards. Vessel had chaffed wired and unsecured heater IWA generator. Secure heater and replace wiring. 46CFR111.01(a)(2)

Approved hand portable fire extinguishers shall be installed on all vessels. Vessels #17 fire extinguisher not properly mounted. Mount extinguishers in approved brackets. 46CFR95.15

No repairs or alterations affecting the safety of vessel with regard to the hull, machinery or equipment shall be mad with out the knowledge of the [name withheld] walkway deck entry to crane was found to be wasted. Submit repair proposal.

Each vessel must carry lifebuoys approved under approval series 160.150 as follows. Lifebuoys retroreflective tape was extremely faded; replace.

The lights prescribed in these Rules shall have an intensity as specified in Annex I, to these Rules so as to be visible from the following minimum ranges: (a) In a vessel of 50 meters or more in length: (ii) A sidelight, 3 miles. Vessel's starboard sidelight was faded; replace.

Operational readiness. Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Life raft shell is damaged.

The purpose of this subpart is to establish requirements for oil spill response plans for nontank vessels. The development of a nontank vessel response plan prepares the vessel's crew and ship management to respond to an oil spill. NTVRP states when operating in Charleston COTP zone oil spill removal equipment will be provided for Average Most Probable Discharge, provide list showing all required equipment is available. Each cargo vessel must carry an immersion suit approved under approval series 160.171 of an appropriate size for each person on board. Immersion suit were not of appropriate size for crew is present.

(1) If your vessel's Certificate of Inspection is renewed annually, the marine inspector must check the settings of the safety or relief valves on all pressure vessels, except cargo tanks, at each inspection for certification. (2) If your vessel's Certificate of Inspection is renewed less often than annually, the marine inspector must check the settings of the safety or relief valves on all pressure vessels, except cargo tanks, twice every 5 years. No more than 3 years may elapse between any check and its immediate predecessor Relief valves have not been tested.

A vessel owner or operator required by §155.5035 to have a vessel response plan (VRP) must conduct exercises as necessary to ensure that the VRP will function in an emergency. Vessel owners or operators must include both announced and unannounced exercises; and A vessel owner or operator must comply with the VRP exercise requirements of 33 CFR 155.1060. Missing quarterly QI drill, OSRO annual drill and triennial exercise.

he emergency means of shutdown required by §§154.550 and 155.780 of this chapter, as applicable, is in position and operable. Fuel transfer pump shut down button was stuck with corrosion.

The master or person in charge shall ensure that tables indicating the maximum safe working loads for the various working angles of the boom, where the boom is rated at varying capacities depending on the radius, and the maximum and minimum radius at which the boom may be safely used, are conspicuously posted near the controls and are visible to the operator when working the crane. Was missing from crane operations location.

MSD must be free of design defects such as rough or sharp edges that may cause bodily injuries or that would allow toxic substances to escape to the interior of the vessel. MSD tank has holes in the cover and possible other repairs not per manufacturers approval.

Sufficient illumination must be provided by the emergency lighting source under emergency conditions to effect damage control procedures and to provide for safe egress from each space.

Liferafts shall be installed in a float free configuration. Liferaft was not installed to be float free.

Fire hazards shall be minimized. Main diesel generator turbo was found with excessive oil coating the turbo.

Barge does not have survival craft onboard. Barge is required to carry survival craft in accordance with 46 CFR 199.640(a) for 10 industrial personnel.
