An inspection either general or partial depending upon the circumstances shall be made whenever any important repairs or alterations are undertaken. Two areas of corrosion were identified during gauging (ref. VAS drawing# “MnDk2”) the two areas were located on the Port aft outboard and Starboard forward outboard lashing rails. Provide repair proposal to the satisfaction of the attending Marine inspector. Complete approved repairs to the satisfaction of attending Marine inspector.

Following damage survey submit a repair proposal to be reviewed and approved by the attending USCG Marine Inspector. Once repair plan is approved by USCG Marine Inspector complete repairs to the satisfaction of the attending USCG Marine Inspector. 46CFR91.30-1(a)

Vessel shall comply with all aspects of approved mitigation plan, communication plan, and loading plan while operating with temporary repairs. Additionally, only the minimum amount of operations determined to be essential shall be conducted. Vessel shall be replaced with a properly certificated alternative vessel and be sent for permanent repairs no later than 05APR2021. Permanent repair proposals shall be submitted to the attending USCG Marine Inspector for review and approval.

A linear tear, approximately 10" in length and 2" wide, was found on the port stern rake wrapper just below the knuckle. Submit repair plan for permanent repairs to cognizant OCMI. Upon completion of the permanent repairs, the repair shall be verified satisfactory by attending USCG Marine Inspector.

The centerline longitudinal bulkhead in No. 5 void port between Frame Nos. 27-29, was found inset in way of connection to the bottom shell plating. Submit repair proposal to OCMI and make permanent repairs to bulkhead.

The deck edge connection weld was found fractured and weeping water in the port aft rake. Submit repair proposal to OCMI and make permanent repairs to the deck edge connection in the port aft rake.

Prove proper operation of port and starboard running lights.

Complete annual inspection in accordance with 46CFR91.27-1(a) and 46CFR 91.27-5

Prove proper operation of the fuel transfer system.

Replace ruptured flame screens for cargo tanks #5P & 4S

Replace broken exhaust piping on inboard side the port engine prior to operating the engine.

Label and adjust emergency F/O shut off valve.

Prove proper operation of cargo pump relief valves prior to conducting cargo off load operations.

Secure electrical conduit on deck starboard side beneath forward house bulkhead

Renew all tank hatch covers.

Prove proper operation of the cargo pump emergency shutdown located on the cargo deck. Barge is restricted from conducting transfers until corrected.

Renew identified crack welds between the identified stiffener/hull plating in the #1 port tank.

Conduct fit up, back gouge, and final air test of the port stern rake insert to the satisfaction of an attending ABS surveyor or USCG MI.

Complete Certificate of Inspection exam prior to conducting cargo or transfer operations.

Provide one ringbuoy at bow of barge IAW 46 CFR 199.640(i) (2).

Renew port navigation light power switch.

Found 4 of 7 portable B-II fire extinguishers were overdue for service. Required operator to have a minium of 5 serviced fire extinquishers prior to conducting cargo ops.

Required operator to stencil correct vessel name/port. Life ring still had old vessel name.

Starboard nav light inoperable. Required operator to repair nav light. Restricted vessel operations to daylight hours.

Required operator to repair emergency lights over port and starboard MDEs. Both lights were inoperable.

Required operator to stencil emergency shutdowns in visible red block lettering. Existing placards were signifcantly faded.

Provide plans for reinstallation of new generator. Vessel is restricted to LA-LB Harbors until new generator is installed.

Blank of block 6 of space heater's distribution panel in engine room.

MAKE AVAILABLE FOR INSPECTION PORT SLOPE SHEET EXTERNAL BOW RAKE IN WAY OF DAMAGE FROM COLLISION ON 9-29-97. THIS ITEM TO BE COMPLETED PRIOR TO LOADING CARGO BUT IN NO CASE LATER THAN 7 OCT 97

PROVE PROPER OPERATION OF CARGO PUMP EMERGENCY STOPS.

PROVE PROPER OPERATION OF GENERATOR AND ALL ALARMS.

THERMAL FLUID HEATER NOT TO BE PLACED IN OPERATION UNTIL HYDROED TO 337.5 PSI AND FULL OPERATIONAL TEST CONDUCTED IN PRESENCE OF USCG MARINE INSPECTOR.

OBTAIN A CURRENT USCG DOCUMENT STICKER AND NOTIFY THIS OFFICE BY LETTER WHEN STICKER IS ACTUALLY RECEIVED.
