The number of penetrations in a watertight bulkhead must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. WTB at frame 16 is not watertight. Make bulkhead penetrations watertight to the satisfaction of the attending marine inspector.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Galley ventilation hood fan and ducting were coated in grease. Filter was not present for galley range ventilation assembly. Clean hood assembly, fan, and ducting from excess grease and install filter to the satisfaction of the attending marine inspector.

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. Unless otherwise stated in this section, each item of equipment carried, whether required under this section or not, must be secured by lashings, stored in lockers, compartments, brackets, or have equivalent mounting or storage arrangements that do not: (1) Reduce survival craft capacity; (2) Reduce space available to the occupants; (3) Interfere with launching, recovery, or rescue operations; or (4) Adversely affect seaworthiness of the survival craft. Port upper and both starboard life floats securing straps for pendants, lifelines, and float net were wasted beyond repair. Vessel operator has a spare (12) person life float that will be installed on the port side with a (15) person life float. Float free link is a 21 or more persons. Vessel is restricted to 27 persons on board until life floats are serviced to satisfaction of the attending marine inspector.

The float-free link required by § 180.130(a)(1) must be: (1) Certified to meet § 160.073 in subchapter Q of this chapter, or other standard specified by the Commandant. Both life float stacks were missing manufacturer certification tag. Replace both float-free linkage assemblies to the satisfaction of the attending marine inspector.

Submit Repair proposal for hull from frames 6-17 (Galley bilge/crew accommodation space) centerline from keel on both PORT/STBD sides. Repair for credit dry dock. 46CFR177.01-5

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from machinery failure. While underway, vessel crew observed the shaft from 01 of the vessel’s 04 main propulsion engines moving in and out of the vessel's stern tube. Diagnose shaft failure and submit comprehensive repair proposal to this office via e-mail for review.

On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. Public address speaker on aft deck not functional. Repair or replace and demonstrate proper operation.

Vessel installed new engine control system. Limited DVTP/QFAs were provided on board but were not approved by MSC and did not represent the installed system (test procedures listed functions not installed and installation did not match referenced control box ports/functions). In accordance with CVC Policy 17-07, submit the following for review: (1) A limited QFA of the vessel’s propulsion engine control system (throttle), documenting system compliance to CVC Policy 17-07. The limited QFA should only consider loss of power to the control system. (2) A propulsion control drawing should be submitted with an installation/operational test protocol. This test protocol is an abridged document intended to show compliance with 46 CFR.184.620. Following MSC review, demonstrate power loss functions in accordance with approved procedures in the prsence of an attending USCG marine inspector.

Vessel was not issued a Load Line Exemption Certificate prior to an international voyage.

The inspected vessel does not have an installed Digital Selective Calling (DSC) radio function installed. Properly install radio function and provide proof.

The door alarm at the operating station does not signal for the Engine Room and Lazerrete opening. Properly install door alarms to alert the operating station when these doors are open/closed.
