A transfer is considered to begin when the person in charge...(i) each transfer hose has no unrepaired loose covers, kinds, bulges, soft spots, or any other defect which would permit the discharge of oil or hazardous material through the hose material. PSCO observed soft spot and/or temporary repair on cargo hose connected between #1 Starboard and #6 Port manifold. 33CFR156.120

The conditions of the ship and it's equipment shall be maintained to conform with the provisions of the code to ensure that the ship will remain fit to proceed to sea without danger to the ship or persons on board or without presenting an unreasonable threat of harm to the marine environment. PSCO observed cargo dripping from the 8P Manifold flange at a rate of 12 drips per minute. 74SOLAS(20)VII/10.1 IBC Code 1.5.3.1

"A" CLASS DIVISIONS ARE THOSE DIVISIONS FORMED BY BULKHEADS AND DECKS WHICH COMPLY WITH THE FOLLOWING: THEY SHALL BE CONSTRUCTED AS TO BE CAPABLE OF PREVENTING THE PASSAGE OF SMOKE AND FLAME. PSCO OBSERVED THREE DOORS THAT WERE NOT GAS TIGHT-THE CO2 ROOM DOOR AND TWO DOORS LEADING INTO THE ENGINE ROOM. AIRFLOW WAS DETECTED AROUND THE PERIMETER OF THE DOORS WHILE IN THE CLOSED POSITIONS.

ALL ELECTRICAL APPARATUS SHALL BE SO CONSTRUCTED AND SO INSTALLED AS NOT TO CAUSE INJURY WHEN HANDLED OR TOUCHED IN THE NORMAL MANNER. PSCO OBSERVED A MODIFIED SWITCH IN THE AIR CONDITIONING FLAT WITH EXPOSED ELECTRICAL WIRING.

IN MEETING THESE REQUIREMENTS, THE COMPANY SHOULD ENSURE THAT: INSPECTIONS ARE HELD AT APPROPRIATE INTERVALS; ANY NON-CONFORMITY IS REPORTED, WITH ITS POSSIBLE CAUSE, IF KNOWN; APPROPRIATE CORRECTIVE ACTION IS TAKEN; AND RECORDS OF THESE ACTIVITIES ARE MAINTAINED. THE TECHNICAL AND/OR OPERATIONAL NATURE OF DEFICIENCIES 02-06 PROVIDE OBJECTIVE EVIDENCE OF A SERIOUS FAILURE OF THE ISM CODE IMPLEMENTATION. AN EXTERNAL AUDIT REPORT SATISFACTORY TO THE OCMI MUST BE SUBMITTED PRIOR TO CLEARANCE OF THIS DEFICIENCY AND RELEASE FROM DETENTION. 74 SOLAS (20)IX/3.1, ISM 10.2

SHIPS CARRYING TOXIC OR FLAMMABLE PRODUCTS OR BOTH SHALL BE EQUIPPED WITH AT LEAST TWO INSTRUMENTS DESIGNED AND CALIBRATED FOR TESTING THE SPECIFIC VAPOURS IN QUESTION. PSCO OBSERVED THAT THE VESSEL DID NOT CARRY ANY INSTRUMENTS TO MEASURE TOXIC VAPORS OF VINYL ACETATE, A CARGO CURRENTLY LOADED ON-BOARD. 74 SOLAS (01)VII/10.1, IBC 13.2.2

THE CONDITION OF THE SHIP AND ITS EQUIPMENT SHALL BE MAINTAINED TO CONFORM WITH THE PROVISIONS OF THE PRESENT REGULATIONS TO ENSURE THAT THE SHIP IN ALL RESPECTS WILL REMAIN FIT TO PROCEED TO SEA WITHOUT DANGER TO THE SHIP OR PERSONS ON BOARD. PSCO OBSERVED TEMPORARY REPAIRS AND PIPE CLAMPS IN 09 LOCATIONS ALONG THE VAPOR COLLECTION SYSTEM "C" LINE. 74 SOLAS (20)I/II(a),IBC 1.5.3.1

A TRANSFER IS CONSIDERED TO BEGIN WHEN THE PERSON IN CHARGE...NO PERSON SHALL CONDUCT AN OIL OR HAZARDOUS MATERIAL TRANSFER OPERATION UNLESS: (f) THE END OF EACH HOSE AND LOADING ARM THAT IS NOT CONNECTED FOR THE TRANSFER OF OIL OR HAZARDOUS MATERIAL IS BLANKED OFF. PSCO OBSERVED 02 HOSES NOT IN USE AND NOT BLANKED OFF ON DECK. 33 CFR 156.120

PROVISION SHALL BE MADE TO ENSURE THAT THE LIQUID HEAD IN ANY TANK DOES NOT EXCEED THE DESIGN HEAD OF THE TANK. PSCO OBSERVED CARGO TANKS 9 STARBOARD, 10 PORT, AND 10 STARBOARD ABOVE 98%, WITH 10 STARBOARD AT 99.8%, VISUALLY OBSERVED FROM CARGO CONTROL ROOM. OBSERVATION MADE POST OVERFLOW CASUALTY RESULTING IN CARGO RELEASE ONTO THE DECK VIA THE VAPOR LINE. 74SOLAS(01)VII/10.1,IBC 8.2.2

ELECTRICAL INSTALLATIONS SHALL BE SUCH THAT: ELECTRICAL SERVICES ESSENTIAL FOR SAFETY WILL BE ENSURED UNDER VARIOUS EMERGENCY CONDITIONS. PSCO WITNESSED ALL BUT ONE EMERGENCY LIGHT NOT ILLUMINATED ON ACCOMODATION EXTERIOR DURING TEST. 74 SOLAS (04) II-1/40.1.2

THE CONDITION OF THE SHIP AND ITS EQUIPMENT SHALL BE MAINTAINED TO CONFORM WITH THE PROVISIONS OF THE PRESENT REGULATIONS TO ENSURE THAT THE SHIP WILL REMAIN FIT TO PROCEED TO SEA. PSCO OBSERVED FRAMO PUMPS ON 12P, 13P, 13S, 11P, AND 9P LEAKING HYDRAULIC OIL. ADDITIONALLY, PSCO OBSERVED SIGNIFICANT HYDRAULIC OIL ACCUMULATION UNDER FRAMO POWER PACKS. ALSO, FRAMO CONTROL UNIT IN CCR SHOWS WITH "PROTECTION PRESSURE" ALARM DUE TO FAULTY SENSOR. 74 SOLAS (20) I/11A

LIFEBUOYS COMPLYING WITH THE REQUIREMENTS OF PARAGRAPH 2.1.1 OF THE CODE SHALL BE: SO DISTRIBUTED AS TO BE READILY AVAILABLE ON BOTH SIDES OF THE SHIP. PSCO OBSERVED LIFEBUOY ON PORT MANIFOLD MISSING. VERIFIED ITS LOCATION ON GENERAL ARRANGEMENT. 74 SOLAS (20) III/7.1.1

EACH TANKSHIP WITH A VAPOR COLLECTION SYSTEM MUST BE FITTED WITH A PRESSURE SENSING DEVICE. 46CFR39, 2013 PSCO OBSERVED PRESSURE SENSORS ON VAPOR LINES “A” AND “D” INOPERABLE. VAPOR LINE “A” WAS CURRENTLY ALLIGNED FORVAPOR CONNECTION ON 11P, 9P, 14P, 12P, 10P, 13S, AND 11S

EACH VESSEL VAPOR CONNECTION FLANGE MUST HAVE A PERMANENT STUD. 46CFR39, 2001(j) PSCO OBSERVED VAPOR LINE “D” ON STARBOARD MANIFOLD, WITHOUT STUD.

Electrical installations shall be such as to minimize the risk of fire and explosion from flammable products. PSCO observed tank radars on 7S, 9S, 11S, 13P, 14P, with exposed wiring. Additionally, PSCO observed H/L sensor on 11S with exposed wiring. 74Solas (20) VII/10.1 IBC 10.1.2.1

The fire resistance of doors shall be equivalent to that of the division in which it is fitted.. doors required to be self closing shall not be fitted with hold back hooks. PSCO observed inoperable self closing doors in ladder well down from CCR, ECR, and galley. Additionally class “b” fire door into CCR was unable to be closed. PSCO observed fresh water hose run through class “a” door leading to steering preventing closure. 74Solas (01) II-2/ 47

The company and the ship shall comply with the requirement of the ISM code. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations. The technical and operational nature of the deficiencies 02-13 provide objective evidence of a serious failure, or lack of effectiveness, of the implementation of the ISM Code. A safety management audit carried out by the administration, or RO, with a corresponding report of findings satisfactory to the OCMI must be submitted prior to clearance of the deficiency and release from detention. 74 SOLAS (20) IX/3.1 ISM Code 10.1

The total suction head and net positive suction head of the pump shall be such that the requirements of paragraphs 3.3.2, 3.3.2.1, 3.3.2.2, and 4.2 of these regulations shall be obtained under all conditions of trim, list, roll, and pitch likely to be encountered in service. PSCO observed emergency fire pump unable to obtain suction. Ships crew stated that that the pump does not work when in ballast condition. 74 SOLAS (01) II-2/4.3.3.2.5

Rescue boats shall be stowed in a state of continuous readiness for launching in not more than 5 minutes. PSCO observed rescue boat fail to start and run within 5 minutes. 74 SOLAS (20) III/14

Performance of crew members assigned fire-fighting duties shall be periodically evaluated by conducting on-board training and drills to identify area in need of improvement, to ensure competency in fire-fighting skills is maintained, and to ensure the operational readiness of fire-fighting organization. PSCO observed 2 failed fire drills at midship stores. Observed crew unfamiliarity with fire-fighting equipment leading to unsafe conditions for fire team. A satisfactory fire drill is required prior to clearing this deficiency in the presence of a Coast Guard PSCO. 74 SOLAS (20) II-2/16.2.2.3

Alternatively, pressure sensors may be fitted on each deck with a monitoring system in the ships cargo control room. PSCO observed audible and visual alarm systems turned off, due to constant alarms. 74 SOLAS (01) II-2/59.1.2.3

The company should identify equipment and techincal systems the sudden operational failure of which may result in hazardous situations. PSCO observed numerious failures of cargo monitoring deficiencies from the exam on 31Jan2025: #1, #2, #3; the exam on 02Feb2025: #1, #2, #3; and the exam on 13Feb2025: #10, #11, #12. Provide objective evidence of a failure of the implementation of the ISM Code with regard to cargo handling and monitoring systems. An internal audit report satisfactory to the OCMI must be submitted within 90 days prior to the clearance of this deficiency. 74 SOLAS (20) IX/3.1 ISM Code 10.3

Alternatively, pressure sensors may be fitted in each tank protected by the arrangements required in 1.2.2. PSCO observed inoperable tank pressure senors on 2p, 7s, % 9s. Vessel relies on pressure senors as alternative means under 74 SOLAS II-2/59.12. PSCO observed additional pressure senor on 5s/p defective. due 01Mar2025 IAW flag letter: RCS 13979-01-2025. 74 SOLAS (01) II-2/59.1.2.3.

Each cargo tank of a tankship must be equipped with intrinsically safe high-level alarm and a tank overfill alarm. PSCO observed inoperable overfill alarm on 5p and high level and overfill alarm on 7p out on deck. Repair due in accordance with flag dispensation RCS 13979-01-277-2025. 46 CFR 39.2007(a)

THE ARRANGEMENT FOR PROVIDING FOAM SHALL BE CAPABLE OF DELIVERING FOAM TO THE ENTIRE CARGO TANK DECKS AREA. PSCO OBSERVED LEAK FROM FOAM LINE OF APPROX. 2 GALLONS/MINUTE UNDER MANIFOLD, LINE WAS FITTED WITH TEMPORARY REPAIR WHICH WAS NOT HOLDING. 74SOLAS(01) II-2/61.1

Suitable instructions and component spares for testing and mainanence shall be provided. PSCO observed ships crew open heat detectors with heat gun. The authorized heat detector testing mechanisom requires a power supply not available on the vessel.

ELETRICAL EQUIPMENT, CABLES, AND WIRING SHALL NOT BE INSTALLED IN THE HAZARDOUS LOCATIONS UNLESS IT CONFORMS WITH THE STANDARDS NOT INFERIOR TO THOSE ACCEPTABLE TO THE ORGANIZATION. PSCO OBSERVED EXPOSED WIRING ON MULTIPLE TANK GAUGING SYSTEMS ON THE CARGO DECK, AND EXPOSED WIRING AT CABLE PENETRATION IN FRAMO POWER PACK ROOM. 74SOLAS(20)VII/10.1 IBC 10.1.4

Where "A" class divisions are penetrated for the passage of electric cables, pipes, trunks, ducts, etc., or for girders, beams or other structural members, arrangements shall be made to ensure that the fire resistance is not impaired. PSCO observed multiple electrical cable penetrations through "A" class boundaries with missing or cracked insulation in the bridge electrical locker, the passage above the wheel house door, the cable trunk on the "C" deck, and the penetration leading into the wheelhouse from the port bridge wing. 74SOLAS(2001) II-2 18.1.1

Alternatively pressure sensors may be fitted in each tank with a monitoring system in the ship's cargo control room. PSCO observed inoperable tak pressure sensirs on 2P, 7S, & 9S. VSL relies on pressure sensors as alternative means under 74 SOLAS (01) II-2 / 59.1

ALTERNATIVELY, PRESSURE SENSORS MAY BE FITTED IN EACH TANK WITH A MONITORING SYSTEM IN THE SHIPS CARGO CONTROL ROOM. PSCO OBSERVED ERRORS ON COT 10P, 11P 14P DISPLAY IN CCR ADDITIONALLY, AUDIBLE ALARMS TO THE SYSTEM HAVE BEEN SECURED LOCALLY DUE TO CONSTANT ALARMS. 74SOLAS(01)II-2/59.1.2.3

EACH CARGO TANK OF A TANKSHIP MUST BE EQUIPPED WITH AN INTRINSICALLY SAFE HIGH-LEVEL ALARM AND A TANK OVERFILL ALARM. PSCO OBSERVED INOPERABLE VISUAL HIGH-LEVEL ALARM AND TANK OVERFILL ALARM OUT ON DECK. 46CFR39.2007(a)

SUITABLY MARKED DECONTAMINATION SHOWERS AND EYEWASH SHALL BE AVAILABLE ON DECK IN CONVENIENT LOCATIONS. THE SHOWERS AND EYEWASH SHALL BE OPERABLE IN ALL AMBIENT CONDITIONS. PSCO OBSERVED INADEQUATE PRESSURE OF THE SHOWER AND EYEWASH STATIONS. 74SOLAS(20)VII/10.1

The ring, lines and letters shall be painted in white or yellow on a dark ground or in black on a light ground. They shall be permanently marked on the sides of the ships. PSCO observed starboard side plimsoll line was not painted. ICLLC REG. 8

Measures shall be taken to the satisfaction of the Administration to ensure that the equipment is functioning in a reliable manner and that satisfactory arrangements are made for regular inspections and routine tests to ensure continuous reliable operations. PSCO observed inoperable engine gauge cluster on the starboard side main generator. 74 SOLAS(CONS. 2009)II-1/46.2

Provisions shall be made for marking each liferaft with the name and port of registry of the ship to which is is to be fitted, in such a form that the ship identification can be changed at any time without opening the container. PSCO observed starboard liferaft did not display the name and port of registry of the vessel. 74 SOLAS(CONS. 2020)III/31.1.2.2, LSA Code 4.2.7.2

On-board training in the use of the ship's life-saving appliances shall be given as soon as possible. PSCO observed Oiler was not familiar with use of anti-exposure suit. 74 SOLAS(CONS. 2020)III/19.4.1

Each lifebuoy shall be marked in block capitals of the Roman alphabet with the name and port of registry of the ship on which it is carried. PSCO observed portside lifebuoy did not contain name and port of registry of the vessel and had inadequate retro-reflective material. 74 SOLAS(CONS. 2020)III/7.1.4

In meeting these requirements, the company should ensure that... .2 any non-conformity is reported, with its possible cause, if known; .3 appropriate corrective action is taken. 02, 04, 05, 06, 07, 08, 09 deficiencies below provide objective evidence of a serious failure of the implementation of the ISM Code. An external audit report is recommended. 74 SOLAS(CONS. 2020)IX/3.1, ISM CODE 10

The electrical power available shall be sufficient to supply all thos services that are essential for safety in an emergency. PSOO observed #2 Start Air compressor was removed. According to ships diagrams was wired to emergency switch board. 74 SOLAS(Cons. 2004) II-1/43.2

Machinery Spaces of category A shall be adequately ventilated so as to ensure that when machinery or boilers therein are operating at full power in all weather condtions...During inspection of engine room, there was temporary ventilation to the BWTS. Class shall verify proper operation of BWTS without portable ventilation. 74 SOLAS(Cons. 2004)II-1/35

For the purpose of adequately controlling cargo, cargo transfer systems shall be provided with...one stop valve capable of being manually operated on each tank filling and discharge line, located near tank penetration...PSCO observed during deck walk, a hydraulic oil leak near the #10 STBD FRAMO pump. Crew stated it was from the gauge area of the FRAMO. 74 SOLAS(Cons. 2020)II-2/4.1.1

Maintenance, testing and inspections shall be carried out based on the guidelines developed by the organization. PSCO observed a portable fire extinguisher near annex II locker had not been inspected since July but marked on SMS procedures as completed by crew. 74 SOLAS(CONS. 2020)II-2/2.2.1

The rescue baot embarkation and launching arrangements shall be such that the rescue boat can be boarded and launched in the shortest time possible. PSCO observed inoperable rescue boat davit. See Panama Flag Dispensation SEG-067568. 74SOLAS(01) III/17.1 40AC

Fire fighting systems appliances shall be kept in working order and readily availible for immediate use. PSCO observed pin hole leak on fire main supply pipe to fire hydrant on port bridge wing. 74SOLAS(14)II-2/14.2.1.2 40ac

Every tanker shall be provided with the means to enable the crew to gain safe access to the bow even in severe weather conditions. PSCO observed excessivly wasted deck grating near midship deck crane. 74SOLAS(01) II-1/3-3.2 40ac

The venting arrangements shall be so designed and operated as to ensure that neither pressure nor vacuum in cargo tanks shall exceed design parameters and be such as to provide for: alternatively, pressure sensors may be fitted in each tank protected by the arrangement required in 1.2.2, with a monitoring system in the ship's cargo control room or the position from which cargo operations are normally carried out. PSCO observed non-operational audible alarm for cargo tank pressure sensors.

Unless expressly provided otherwise or unless, in the opinion of the Administration having regard to theparticular voyages on which the ship is constantly engaged, other requirements are appropriate, all life-savingappliances prescribed in this part shall: early marked with approval information including the Administration which approved it, andany operational restrictions; PSCO observed lifejackets missing approval markings.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO's observed significant wastage and leaks on the following systems: hydraulic return line above tanks 12 & 13, cargo steam line above tank 6, main cargo common line at port manifold.

Satisfactory means (in the form of guard rails, life lines, gangways, or underdeck passages,) shall be provided for the protections of the crew in getting to and from their quarters, the machinery space and all other parts used in the necessary work of the ship. PSCO observed the STBD aft P/V bank deck grating severely corroded.

The Secretary may order any vessel, in a port or place subject to the jurisdiction of the United States or in the navigable waters of the United States, to operate or anchor in a manner the Secretary directs if - (1) the Secretary has reasonable cause to believe such vessel does not comply with any regulation issued under section 70034 or any other applicable law or treaty; (2) the Secretary determines such vessel does not satisfy the conditions for port entry set forth in section 70021 or 70022 of this title; or (3) by reason of weather, visibility, sea conditions, port congestion, other hazardous circumstances, or the condition of such vessel, the Secretary is satisfied such direction is justified in the interest of safety. PSCO observed insufficient hot water supply to the hospital space. (Approximately 1 gallon per mintue). A very slow pour from faucet and one small nozzle of the shower was passing water. The superintendant was storing his luggage in the hospital space as well.

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met: means shall be provided to control leaks of flammable liquids. PSCO observed three high pressure fuel pumps on the main diesel engine leaking fuel oil, also the #3 high pressure fuel line, as well as a substantial accumulation of MGO on the main engine. 74SOLAS(01)II-2/15.4
