The term flame screen means a fitted single screen of corrosion resistant wire of at least 30 by 30 mes or two fitted screens, both of corrosion resistant wire, of at least 20 by 20 mesh, spaced not less than 1/2 inch or more than 1 1/2 inches apart. Flame screens to multiple pressure/vacuum valves and wing ballast tank found with missing flame screens. (PVs 4s, 1p, and WBP1)

Resolution: Flame screens were replaced with new in PV valves 4s, 1p and Water Ballast tank 1p.

Carbon dioxide systems installed on or after 1 October 1994 shall have two controls located inside a release box clearly identified for the particular space. If the box containing the controls is to be locked, a key to the box shall be in a break-glass-type enclosure conspicuously located adjacent to the box. Resolution: Survey report states: New control boxes with new locks and key for Fixed CO2 Fire Extinguishing Systems station have been verified installed into position. The lock/unlock operation ability of provided keys were tried out in order.
