Vessels certificated to carry cargoes with a flash point below 140 degrees may only have approved intrinsically safe, explosion proof, or purged and pressurized electrical equipment in areas designated as Class 1 Division 1 as per 111.105-31(1). All components of the system must be maintained in the condition during initial installation as approved by required plan review per 111.105-11 and 110.25-1(j). Found significant chaffing on electrical cables for high level alarms at midships. Replace electrical cables or repair in an approved method as per 111.01-1(a)(3). Vessel is restricted to carrying grade D and E cargoes with a flash point of 140 degrees and above.

Emergency shutdown failed to shutdown the cargo pump engine. After multiple attempts local personel effected repairs and defiency was corrected on the spot.

Resolution: The issue was corrected by adjusting the wire pull system and tightening bolts. Upon completion of repairs system was retested and found satifactory by attending inspectors.

Emergency shut-off for the cargo pump's prime mover (diesel engine) did not operate satisfactorily. There was excessive cable on the AMOT shut-off valve that did not allow the emergency shut-off pull cable to fully trip the AMOT valve. Also, there was a cotter pin missing from the shut-off lever.
