Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Crew members assigned to the starboard side aft liferaft station were unfamiliar with liferaft launching procedures. 74 SOLAS III/19.2.1

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed Lifeboat #2 rudder was unable to turn to starboard using the primary means of steering. 74 SOLAS (20) III/20.2 Code 17

The means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections. The marking must enable passengers to identify the routes of escape and readily identify the escape exits. Additionally, escape route signs and fire equipment location markings shall be of photoluminescent material or marked by lighting. PSCO observed Low Location lighting was not operational outside of the greenhouse spa and directly outside of the garbage handling room. 74 SOLAS(01) II-2/28.1.10 Code 10

Every ship of 100 gross tonnage and above, and every ship which is certified to carry 15 or more persons, and fixed or floating platforms shall carry a garbage management plan which the crew shall follow. PSCO observed plastic bottles discarded in a combustible waste receptacle designated for aluminum only in the aft main galley on deck two. MARPOL 2017 Annex V/ 10.2 Code 10

PSCO observed multiple crew members assigned to Liferaft teams who were unable to demonstrate satisfactory knowledge of procedures required for launching liferafts. 74 SOLAS (20) III/10.2.1 Code 10

For determining the appropriate fire integrity standards to be applied to boundaries between adjacent spaces, such spaces are classified according to their fire risk as showing in categories (1) to (14) below. A/C station 1/2, on space deck 1/ MVZ 2, is a category 10 space where the storage of combustibles is prohibited. PSCO observed storage of cardboard boxes and carpeting within the space. 74 SOLAS 01 II-2/26.2.2 Code 10

Escape route markings must enable passengers to identify the routes of escape and readily identify the escape exits. PSCOs observed unmarked routes of escape in three different spaces. Space 1) both exits within the Thermal Suite unmarked and not readily identifiable. Space 2) both exits within the Neptune Lounge unmarked and not readily identifiable. Space 3) both exits within the Deck 3, aft portion of the theater (aft exits) unmarked and not readily identifiable. 74 SOLAS 01 II-2/28.1.10 Code 10

Doorways and corridors and intermediate landings included in the means of escape shall be sized in the same manner as stairways. Shall not be less than 900 mm in clear width. PSCO observed both escape routes from the Hydro Pool obstructed by decorative doors that were very difficult to open due to contact with the ceiling and contact/rubbing with the rug beneath. 74 SOLAS 01 II-2/28.1.5.3 Code 10

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power required by regulation II-1/42 or II-1/43. PSCOs observed 03 embarkation lights not illuminated, one between lifeboat 09&11, Tender 15, and one by lifeboat 01. 74SOLAS(20) III/11.4

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, the approximate time of closure for hinged fire doors shall be no more than 40s and no less than 10s from the beginning of their movement. PSCOs observed 04 fire screen doors in main vertical zones, stairs, and galley boundaries closed too fast (FSD 08.5.4, FSD 01.06.11, FSD 0A.3.79, FSD 0A.1.117). 74SOLAS(01) II-2/30.4.2

...escape route signs and fire equipment location markings shall be of photoluminescent material or marked by lighting. PSCO observed 02 emergency escape route sign lighting was out. One was located forward on Deck A by fire locker A.06.03, and the other inside of the Hudson Room on Deck 3. 74 SOLAS (20) II-2/13.3.2.5.1 17c

Passenger ships carrying more than 36 passengers shall be equipped with an automatic sprinkler, fire detection, and fire alarm system of an approved type complying with the requirements of the Fire Safety Systems Code in all control stations, accommodation and service spaces, including corridors and stairways. PSCO observed broken sprinkler ampule on Deck 6 inside room 6108. 74 SOLAS (01) II-2/6.1.1 17c

Crew members shall be trained to be familiar with the arrangements of the ship as well as the location and operation of any firefighting systems and appliances that they may be called upon to use. 4 mess cooks in the Lido deck and crew mess were unfamiliar with the location of the high pressure water mist activation and emergency power shutoff for the deep fat fryer. 74 SOLAS (20) II-2/15.2.2.1 17c

The public address system shall be clearly audible above the ambient noise in all spaces, and shall be provided with an override function controlled from one location on the navigation bridge. PA system at lifeboat assembly station 16 was inoperable. 74 SOLAS (20) III/6.5.2 16ac

All lifesaving appliances and arrangements shall comply with the applicable requirements of the Code. The name and port of registry of the ship to which the lifeboats shall be marked on each side of the lifeboat's bow in block capitals of the Roman Alphabet. PSCO observed faded vessel name and home port on lifeboat #8. 74 SOLAS (20) III/34 LSA Code 4.4.9.2 40ac

Each emergency generating set arranged to be automatically started shall be equipped with...a second source of energy...for an extra three starts within 30 min. PSCOs observed the emergency generator secondary means of starting was inoperable due to the air starter being inoperable. 74 SOLAS (04) II-1/42.2

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: fire detection and fire alarm systems. PSCO’s observed 02 heat detectors that were non-operational. PSCO’s were informed additional parts were needed to conduct repair. Heat detectors were: freezing room 8 and forward mooring deck SV aft. 74 SOLAS 20 II-2/14.2.1.1.2

The purpose of this regulation is to provide means of escape so that persons on board can safely and swiftly escape to the lifeboat and liferaft embarkation deck. For this purpose, the following functional requirements shall be met… additional aids for escape shall be provided as necessary to ensure accessibility, clear marking, and adequate design for emergency situations. PSCOs observed the Neptune lounge on Deck 7 with no escape signage posted. 74 SOLAS 20 II-2/13.1.3

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. All boat crew assigned to lifeboat 9 were not familiar with procedures for bilge pumping and emergency steering. All boat crew assigned to lifeboat 11 were not familiar with locations of required provisions & equipment. 74SOLAS 20 III/19.2.1

Each emergency generating set arranged to be automatically started shall be equipped with starting devices approved by the Administration with a stored energy capability of at least three consecutive starts. A second source of energy shall be provided for an additional three starts within 30 min unless manual starting can be demonstrated to be effective. The PSCOs observed that the second means of starting the emergency generator was inoperable. 74 SOLAS (20) II-1/44.2 (Code 17ac)

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. PSCOs did not satisfactorily witness the simulated abandonment of the vessel. During the abandon ship drill, crew did not adequately perform the positions to which they were assigned to abandon the vessel. 74 SOLAS (20) III/19.2.1 (Code 17ac)

Safe escape routes shall be provided... PSCOs observed that areas in the galley and shops do not provide passengers or crew immediate and clear notification of a fire in the surrounding space. 74 SOLAS 01 II-2/13.1.1 (Code 16ac)

Doors in escape routes shall in general open in way of the direction of escape. Emergency exit door on Deck 10 starboard side by the kids club does not open. 74 SOLAS (01) II-2/3.3.1.5

Each survival craft shall be stowed in a state of continuous readiness so that two crew members can carry out preparations for embarkation and launching in less than 5 min. Lifeboats #2 and #14 were unable to be launched in 5 min. 74 SOLAS (14) III/ 13.1.3

Before the ship leaves port and at all times during the voyage, all life saving appliances shall be in working order and ready for immediate use. Tender #15 was found with suspected damage to port shaft bearing. Tender can still operate on one shaft. 74 SOLAS (14) Ch III/ 20.2

The master, owner, operator, agent, or person in charge of a vessel equipped with ballast tanks that operates in the waters of the United States must employ one of the following ballast water management methods. Vessel states that they do not discharge water in the U.S. however approved ballast water management plan does not state such. 33 CFR 151.2025 (a)

Tanks, voids and auxiliary machiner spaces having litte or no fire risk. NFPA 30 flammable liquid locker found in category 10 space, 1st deck theater backstage. NFPA 30 lockers should only be used in 11, 12, or 14 spaces. 74 SOLAS (01) II-2/ 9.2.2.3.2.2.10

A main source of power of sufficient capactiry to supply all services mentioned in reg 40.1.1 shall be provided. While ship was running its turbine generator vessel was unable to disconnect from main board without shutting down all ship's generators casuing ship to black out. 2004 SOLAS II-1 /41.1.1

When an exemption is granted to a ship and in accordance with the provisions of the present regulations, a certificate called an Exemption Certificate shall be issued in addition to the certificates prescribed in this paragraph. The PSSC Form P indicates that no Exemptions have been issued, however, three (03) exemptions are issued and effective onboard.

Fire doors in stairway enclosures shall be self-closing and shall have an approximately uniform rate of closure of no more than 40 s and no less than 10 s with the ship in the upright position. FSD 04.5.17 was not self-closing, and FSD 06.1.54 and the door from the spa into the forward stairway enclosure closed in less than 10 s.

Low-location lighting shall be placed at all points of the escape route. LLL is absent in stairwell 12 which forms a means of escape from the storekeeper's office.

LLL should lead to the door handle. The LLL at the door forward of the crew gym forming the means of escape did not lead to the handle.

Furniture in stairway enclosures shall be limited to seating. Chairs were stored in stairwell 2 on deck 1 and ping pong table and luggage racks were stored in staircase 10 on deck 1.

Muster and embarkation stations should be adequately illuminated by lighting supplied from the emergency source of electrical power. 4 upper deck embarkation lights were extinguished.

Vessel did not meet ANOA requirements.

0750 - "A" class divisions are those divisions formed by bulkheads and decks which comply with the following: They shall be so constructed as to be capable of preventing the passage of smoke and flame to the end of the one-hour standard fire test. There are holes drilled through emergency escape door frames in machinery space "A" class division. Door frames for doors FSD 0D.4.03 and FSD 0D.5.01. 74SOLAS(01 Cons)II-2/3.3 16ac

1499 - All the steering gear components shall be of sound and reliable construction. There is hydraulic fluid leaking from the sight glass on the port side azipod steering servo unit 1. 74SOLAS(01Cons)II-1/29.2.1 16ac

0820 - Electrical installations shall be such that the safety of passengers, crew, and ship from electrical hazards will be ensured. In the life jacket storage room located behind the electrical panel in the engine control room, there are no covers on any of the electrical panels backing. 74SOLAS(01Cons)II-1/40.1.3 16ac

0750 - Stairways and ladders shall be arranged to provide ready means of escape to a life boat and life raft embarkation deck from all passenger and crew spaces and from spaces in which the crew is normally employed as per MSC guidelines SOLAS - 25. There is a room in room configuration on deck 2 in the Maitre D hotel office. 74SOLAS(2001)II-2/28.1 16ac

0750 - Fire fighting appliances shall be kept in good working order and readily available for immediate use. 02 Hi-Fog suppression sprinkler nozzles located in the garbage room on Deck A are missing the thermal fluid as required in the heat sensitive glass bulb and the heat detector 27 43 on Deck A is broken. 74SOLAS(2014)II-2/14.2.1.1 10c

Scrubber system installed with no alarm indicator to notify operators on watch. Correct problem to notify engineer on watch when scrubber malfunction like an alarm (Audible/Visible) to prevent burning non-compliant high sulfur content (ECA).

0820-Each power-operated sliding watertight door shall be provided with an audible alarm. The audible alarm was inoperable for water tight door located in fire zone # 5, door # 6. 74SOLAS(04 cons.) CH.II-1/REg.15.7.1.6

0960-There shall not be any doors along any designed escape route which require keys to unlock them when moving in the direction of escape. Fire door C.5.02 located in the laundry space required a key to escape. 74SOLAS(01 cons.) CH.II-2/Reg.28-1.1.8

An adult lifejacket shall be so constructed that the method of securing the lifejacket to the wearer has quick and positive means of closure that do not require tying of knots. Passenger and crew lifejackets require lifejackets to be tied in order to secure. 74SOLAS(14 cons.) CH.III/Reg 34 LSA Code CH. II/2.2.1.5.4

0715 - Detection - Sprinkler detection indicator system not operating properly. System not indicating isolated zone; all zones indicted when one zone is detected.

Manual Fire call box (zone108/63) broken.

Emergency generator inoperable. Installed generator on deck to take the place of the emergency generator temporarily. Repair emergency generator tot he satisfaction of class surveyor within 30 days.

Noted two pin-holes in gelcoat of #12 tender, portside fwd on bottom of hull, and on lifeboat #6 approx. 18 inches above waterline 2" gouge through gelcoat into fiber layers.

Found eye wash bottles in the machinery spaces missing on racks. 1st Engineer stated all on order & have not arrived. Coming from a reliable source, eye wash bottles were being snatch by crew members for drinking. Two bottles were replaced brought up from hospital spare lockers to meet requirements.

Electrical box overhead located in the bow thruster room open without cover, electrical hazard.

Low level lighting found in needed maintenance in the area outside of the main theater port & STBD side due to high traffic area.

Valve handle missing on the discharge side of the topping fire pump located in the machinery space.

Provide flag state acceptance of repair of lifeboat davit number 10 and method to prevent reoccurance of similar casualty.

Renew fuel lines on all tenders to the satisfaction of flag state.

Prove proper operation of all fire shutters on deck 8 and 9. Provide class report to MSO Miami NLT 21 Dec 03.
