A towing vessel must be maintained and operated so that the watertight integrity and stability of the vessel are not compromised. Multiple wastage holes were found in steering compartment space, aft void spaces, and forward hold that breached into other spaces or to the outside of the hull. There was also potential hull cracking discovered below the waterline in the steering compartment and forward void, as evidenced by appearance of water after bilges were emptied. Additionally, there was significant wastage to structural members in steering compartment space, aft void spaces, and forward hold, and engine room. Submit a repair plan to the cognizant OCMI and replace wasted metal and repair as original to the satisfaction of OCMI prior to operating vessel.

Propulsion machinery must be maintained to ensure proper operation. Rudder posts, rudder linkage, through-hull water seals, rudder bearings, and steering gear connections to the hull were all found to be in severe disrepair, with several improper repairs made to the equipment. Repair/replace all steering gear to the satisfaction of the cognizant OCMI.

Each towing vessel must be operated in compliance with applicable sections of the Federal Water Pollution Control Act. Hydraulic oil was observed in the steering space. Locate and repair oil leak to the satisfaction of OCMI prior to operations.

Electrical systems must minimize fire and shock hazards. Properly mount/secure wiring/electrical equipment in the following places: main electrical feeder wire on starboard generator, light above starboard generator, wiring bundle inboard of port main (hanging by a rag), hose/wiring bundle inboard forward of port main (wire bundle hanging from green gage wire), and bilge alarm.

Pressure vessels over 5 cubic feet must have properly operating spring loaded relief valves meeting the requirements of 46 CFR 143.300, must have the Maximum Allowable Working Pressure (MAWP) indicated by a stamp or nameplate, and must be maintained to provide safe operation of the towing vessel and safety of persons onboard. Port air compressor receiver was missing nameplate and had no indication of pressure vessel’s MAWP. Also, the relief valve for the receiver was malfunctioning and there is a vital part of the compressor that is being held in place by a wire tie. Provide MAWP for receiver, repair or replace relief valve, and make proper repairs to the air compressor.

Main engines must be equipped with an RPM gage at the machinery location. Starboard Main Engine RPM gage was not functioning. Repair RPM gage

Additionally, all seals and gaskets must be maintained to prevent leaks of flammable and combustible liquid. There is a fuel hose on the port generator that is made from substandard materials and is beginning to come apart. Replace hose with appropriate hose for system that will withstand vibration.

Additionally, all seals and gaskets must be maintained to prevent leaks of flammable and combustible liquid. There is a fuel leak on port generator. Repair fuel leak.

Each towing vessel must be visible and audible at the operating station. The hydraulic oil low level alarm did not activate at the operating station. Repair alarm.

An existing vessel may be deemed by the OCMI to be in compliance with structural standards unless questioned by the [name withheld] were discovered in the forward hold and lazarette. Submit repair proposal to cognizant OCMI.

Electrical systems and equipment must function properly and minimize fire and shock hazards. Repair open breaker slots and label breaker panel in the wheelhouse.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel. Gauge panel for starboard generator, engine room alarm bell, and bilge alarm bell need to be mounted securely.

An exposed hazard such as gears and rotating machinery must be protected. Repair/replace guards on starboard generator, air compressor, and fuel transfer pump.

All installed bilge piping must have a check/foot valve in each bilge suction. Install check/foot valve in engine room bilge suction line.

Machinery components that exceed 220 degrees celcius must be insulated. Install new exhaust lagging on port main engine.

All pressure vessels must have the MAWP indicated by a stamp, nameplate, or other means visible to the crew. Remove improper MAWP for compressed air pressure vessel.

Each towing vessel must perform the applicable tests in table 143.245(b). Provide evidence of set point testing for all machinery alarms.

Each towing vessel must be equipped with at least two portable battery-powered lights. One must be located in the pilothouse and the other at the access to the engine room. Provide a light for the engine room.

Compressed cylinders must be stowed in a rack in an upright position with the valve protection cap in place. Ensure oxygen and acetylene bottles are stowed securely in a rack.

Each towing vessel must have a reliable means to provide alarm for high bilge levels. Provide high bilge level alarms for forward and stern holds.

Fixed fire pump not outfitted with remote start

Vessel does not have an operable fixed fire detection system on board. Installed system did not function due power supply. Additionally, fire detection sensors in the engine room appeared to have been painted over and will not function properly.

Vessel does not have Type I type life preservers for all personnel on board.

Ensure all fire extinguishers on board are approved and inspected.

Vessel did not have operable Marine Sanitation Device on board.

Vessel had severe exhaust leak on both main engines.

Handrails on push knees are severely wasted.
