Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel experienced a higher than normal temp on the port main due to a faulty injector and broken injector rod.

Resolution: Tech came out and troubleshot the problem and found a faulty injector and a broken injector rod. Tech replaced both all sat. Tech report is located in documents.

The 160-B semi-portable located over the engine room has no tag and the last sticker applied is from 2015. Provide evidence that all required fire extinguishers have been serviced.

No person may intentionally drain oil or hazardous material into the bilge of a towing vessel from any source. For purposes of this section, “oil” has the same meaning as “oil” defined in 33 U.S.C. 1321. Vessel has been draining 10-12 gallons per day of water/fuel mixture from bottom of fuel tanks and dumping it into the bilge. Bilge has approx. 6 inches of fuel/water in bilge. Pump fuel/water mixture to slop tank and clean bilges.

Resolution: Received a report from Terall River Service for the [name withheld] stating the vessel motor sucked up some water into the fuel lines. They drained all fuel lines and changed filters. The engine and generator is now operating properly.
02 - Structural Conditions › N/A - No Subsystem › Hull damage impairing seaworthiness

An existing vessel may be deemed by the OCMI to be in compliance with Subpart B (Structural Standards) provided that the vessel has been both in satisfactory service insofar as structural adequacy is concerned and does not cause the structure of the vessel to be questioned by the OCMI. Marine Inspector found two cracks (approx. 1" each) in vicinity of keel penetration of aft engine room bulkhead, which were leaking water into the engine room from the aft void. Upon inspection of the aft void, it appears that water may also be ingressing from outside the hull, but location could not be determined due to 2' of water filling the void. The cognizant OCMI may require additional examinations of the vessel whenever he or she discovers or suspects damage or deterioration to hull plating or structural members that may affect the seaworthiness or fitness of the vessel. Vessel must undergo a drydock and drydock survey to repair deficiencies identified by the Marine Inspector and to assess any further deterioration to the hull affecting seaworthiness. Survey report and all repairs must be to the satisfaction of the attending USCG Marine Inspector prior to further towing operations. Vessel may transit light-boat to affect repairs consequent to the drydock survey and drydock repairs required.
