Guard rails or bulwarks shall be fitted around all exposed decks. The vessel’s aft freeboard deck (above the z-drive space) is missing bulwarks or guard rails to prevent the falling overboard of the crew. Upon expanding the inspection into the ABS survey for Load Lines (Form LL-11-D) it was found that the vessel is missing the required stanchions and chains at frame 62 required for that purpose.

attending Surveyor.
03 - Water/Weathertight Conditions › N/A - No Subsystem › Cargo & other hatchways

The master shall ensure that all hatches, doors and other openings designed to be water tight or weather tight function properly. Multiple violations including: 1) A 12" deck port cover located on the stbd aft of the vessel was severely deteriorated, siliconed in place and painted over. 2) The starboard-aft most deck hatch granting access to the steering had had a torn gasket incapable of creating a watertight boundary. 3) The deck house and port-side water tight doors failed to close as intended. Daylight was clearly visible around the knife edge seals. 4) The captain's port light dead covers was not closing properly, both port & starboard light dead covers were missing their securing bolts; and stbd dead cover was not installed on its place. 5) A bolted secured hatch located at port side aft, was found to have missing 3 of its securing bolts. 6) A hole was found on the top of the coaming at 12 o'clock position at the port side watertight door.

Each towing vessel must be maintained and operated to minimize fire hazards and ensure all bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Excessive amounts of oily water was found in the Z-drive compartment and engine room bilge.
