Fire dampers serving ventilation systems were tested and found not closing as designed.

Review of annual servicing report for the fix CO2 fire extinguishing system revealed the report did not indicate the CO2 cylinders were weighed as part of the required annual ins

Durintg operational testing, the port side lifeboat emergency battery failed to operate as designed.

The STBD side lifeboat davit HYD SYS was observed leaking from a HYD valve during inspection.

No evidence was provided to demonstrate the required monthly insp of lifeboats and associated davit launching appliance has been conducted.

Both STBD Main Diesel Engines were out of service due to oil analysis results showing high metal content. No evidence of the required 60K hour engine overhaul was provided.

The emergency bilge system failed to operate as designed during operational testing. Repair the emergency bilge system and verify the total system is working as designed.

Lock out / Tag out procedures were found not being followed for Main Engines that were out of service. Failure to properly implement isolation procedures creates an unsafe condition and does not comply with the vessels Safety Management Procedures.

Multiple F/O day tank high level alarms were found in a continuous alarm state, indicating the alarm system was not operating as designed.

The vessel was unable to produce a criticle spare parts list for essential machinery and equipment as required by the vessels SMS.

The fire detector was observed hanging from its mounting location and was not properly secured as designed.

The reduction gear high temp bearing alarm sensor was found in the continuous alarm state, indicating the alarm system was not operating as designed.

No repairs or alterations to the hull or machinery, or to equipment that affects the safety of the OSV, may be made without notice to the cognizant OCMI in the inspection zone where the repairs or alterations are to be made. It was reported that the speed log transducer needed to be replaced. Provide NDT report and description of the repairs.

ALL INSTALLATIONS SHALL BE OF MARINE TYPE ENGIES SUITABLE FOR THE INTENDED SERVICE. VESSEL'S PORT #1MDE LOST A CYLINDER(#1cyl), AND IS NOT OPERATIONAL. REPAIR TO SATISFACTION OF CG [name withheld] TO OPERATING #1MDE IN COMMERCIAL SVC.

The condition of the ship and equipment shall be maintained to conform with regulations and remain fit to proceed to sea. #1 MDE was not operational. – 74 SOLAS (2014 Cons) Chapt. I Reg 11

Before ship leaves port, and at all times, all life saving equipment STBD solenoid valve does not stop engine. Port lifeboat steering inoperable. – 74 SOLAS (2014 Cons) Chapt. III Reg. 20.2

The starboard lifeboat was found improperly stowed in the davit cradle enabling the lifeboat to sit on a steel frame. This condition caused damage to the hull of lifeboat. The starboard lifeboat should be griped into the cradle without contact to the frame and without the use of additional tensioning equipment. The lifeboat shall be repaired. The lifeboat's stowed configuration should match the SOLAS training manual, and its proper recovery and stowage shall be demonstrated to the attending marine inspector to verify proper stowed condition. Under this deficiency and with the expectation that life-rafts remain in service IAW 46 CFR 133.105(b)(2), the vessel is restricted to 46 CFR Subchapter L operations on a domestic voyage only. 46 CFR 199.150(a)(1)

The following subpart generally applies to all (pipe) welding. A pinhole leak at the 12:00 position on the lube oil 8" DIA class II pipe weld seam on the #3 main engine exists. Vessel management shall submit a repair proposal to the below email, outlining the intended method to gouge back to good weld, re-weld position, type of filler rod, amperage intended, and any other relevant facts as determined by the qualified welder. An ABS qualified welder shall be used, their welder qualification on a comparable procedure shall be provided at the email below. Prove the weld is tight by demonstrating the pipe/repaired weld in service, at working pressure, to the attending marine inspector. 46 CFR 56.70-1; Code 16C

Each launching appliance must be approved under 46 CFR Part 160, subpart 160.132 for use with the intended craft. Port and STBD lifeboats found with ratchet straps installed between the “D” ring of the falls fore and aft and their respective davit arms. Provide our office with the approved griping method / arrangement in accordance with 46 CFR 160 or davit manufacturer's design. 46 CFR 199.150(a)(1) ; Code 50C EXTENDED, DUE 27 APR 2023.

Each generator must meet the applicable requirements for construction and testing in section 4-8-3 of the ABS steel vessel rules (46 CFR 110.10-1 incorporated by reference). The generator end on the #1 generator vibrated out a mounting bolt, leading to a misalignment & subsequent overheating of the bearing between the prime mover/generator-end. Vessel management is required to submit a servicing narrative or comparable report from when the generator end was last remounted to the prime mover for review. Vessel is required to replace all effected bearings/couplings, re-align, & prove satisfactory operation of the #1 generator under load to the attending marine inspector. Vessel is not authorized to run the #1 generator until this deficiency is cleared by the attending marine inspector. 46 CFR 111.12-1; Code 50AC

Exhaust pipe must conform to ABYC P-1 and part 1, section 23 of NFPA 302. #3 Generator exhaust pipe, in way of deck 2/3, has a 6"x1/2" crack in the pipe. Accepted temporary repair as a doubler plate w/ complete fillet weld on good metal tested to be gas-tight, and re-attachment of stiffener bracket. Crew is required to monitor the temp repair and report any discrepancies or exhaust leaks to the nearest OCMI. Vessel is directed to make permanent repairs to the pipe within 90 days to satisfaction of Coast Guard and ABS. Due 15 JUNE 2023. 46 CFR 58.10-5(d); Code 705AC

If a vessel is to be constructed, altered, or repaired, the plans, information, and calculations required by this part must be submitted. Vessel installed "Helix" PAMs for plow control/electrical service, provide PAM approvals(03 designed for personnel occupancy), and CG MSC Gen Arrangement installation approval. 46 CFR 127.120 (Original Code 17 prior to depature, Changed to Code 50, obtain PAMS approval in 30 days); Extended def to 27 APR 2023.

Electrical equipment must be installed on a vessel to protect crew members from electrical hazards. Cable for "newly" installed deck generator not properly supported chafe hazard, temporary supports (milk crate) not adequate. Demonstrate overspeed trip, and low lube oil alarm/shutdown on newly installed deck generator. 46 CFR 129.200

Each vessel must meet the requirements of subchapter S of this chapter. Vessel stability computer program not updated for new vessel stability parameters (e.g. lightship) update to ABS Stability book issued 06MAR23. 46 CFR 127.230

Electrical installation on an OSV of less than 6,000GT ITC must comply with 46 CFR Subchapter J. The following plans must be submitted for CG iaw 110.25-3: “Elementary one-line wiring diagrams of the power system, supported, by cable lists, panelboard summaries, and other information including type and size of generators and prime movers. Provide appvd plans; deck equipment/PAMS. 46 CFR 129.110(b), Originally Code 17, Changed to 50C. Extended to 27 APR 2023.

Each vessel must have marine charts of the area to be transited. Vessel does not have paper chart 13205 or chart 12300 for the areas of intended navigation. 33 CFR 164.33

This subpart (submission of electronic advanced notice of arrival) applies to the following vessels that are bound for or departing from ports or places within the navigable waters of the United States: U.S. vessels in commercial service. Vessel arrived to the Sector SENE COTP zone without submission of an E-ANOA and is not employed in the exploration for or in the removal of oil, gas, or mineral resources on the continental shelf. 33 CFR 160.203

ABS Guide for Portable Accommodation Modules (May 2014 Ed.), Section 2/15.15; 46 CFR 161.002; CG-ENG Policy Letter 01-16 (for reference) Each alarm annunciator, fire detector, test station, manual station, and vibrating bell should be approved under 46 CFR 161.002. Vessel’s crew is required to demonstrate functionality of the fire detection system in the 3 PAMS (metal fab PAM, workshop PAM, and plow control PAM), and prove that an indication is provided to the bridge to the attending marine inspector.

Electrical equipment must be installed on a vessel to protect crew members from electrical hazards. Junction box(s) on deck for deck generator not weather tight

Each class 2 or 3 watertight door must be designed, constructed, and tested in accordance with ASTM F 1196. Watertight door in engine room was inoperable and taken apart for maintenance. 46 CFR 170.270(c)

Main and auxiliary machinery shall be tested at each inspection for certification. #2 SSDG was tagged out due to high temperatures, and was unable to run #3SSDG due to engineering dept being unsure of status of maintenance from port engineers. Unable to test over speeds and reverse power relays. Port side MDE cooling pump is being replaced and has not been installed. Found oil leak coming from oil filters on port CPP. 46 CFR 61.20-3(a)

Diesel fuel systems shall prevent mechanical injury and effectively secured against excessive vibration. Fuel leak on the #3 MDE discharge pipe. Fuel line on the #1 SSDG was missing bushing on high pressure fuel line. 46 CFR 56.50-75

Rescue boat must be in good working order. Fire extinguisher bracket was not installed properly or easily accessible. The self-righting inflation bag was tied down and unable to be released. Searchlight did not have end fitting and was unable to be tested. Gas tank had extreme weathering and needs to be replaced. 46 CFR 131.540

Station license will be issued for a term of ten years from the date of original issuance, or renewal. FCC Station License was expired. 47 CFR 80.25

All ships of 3,000 GT and upwards shall have a second 9GHz radar. Starboard radar not operational. 74 SOLAS (14) V/19.2.7.1

Main steering gear must be capable of moving the rudder from 35 degrees on either side. Port steering pump was inoperable and receiving a loss of phase alarm. 46 CFR 58.25-10(b)(2)

The position of the rudder must be shown at the steering station. Starboard rudder angle indicator was 10 degrees off. 46 CFR 113.40-5

Each fire hose must be subjected to maximum test pressure every 12 months. Found three fire hose that leaked. Destroyed in sight of MI and will need to be replaced. 46 CFR 132.350(c)(3)

Each OSV must carry lifebuoys approved under approval series 160.150. Grab lines and stenciling on various ring buoys need to be replaced. Grab lines should be secured around the body of the buoy and evenly distributed. 46 CFR 133.70

Electrical equipment must be installed to protect from hazard. Missing cover on junction box on OWS. Exposed wiring on aft stbd deck from a removed camera. 46 CFR 129.200(a)

Nonmetallic expansion joints must be inspected for signs of excessive wear. #1 MDE and #2 MDE had deteriorated nonmetallic expansion joints. 46 CFR 61.15-12

Vessel shall be fitted with oil filtering equipment. Forward OWS was inoperable. MARPOL (11) I/14.1

Diesel fuel systems shall prevent mechanical injury and effectively secured against excessive vibration. Return and gauge fuel line on #1 and #2 SSDG need to be replaced, due to excessive wear of rubber insulation. 46 CFR 56.50-75

Each class 2 or 3 watertight door must be designed, constructed, and tested in accordance with ASTM F 1196. Watertight door to the bow thruster room was not completely closing and creating a proper watertight seal. Vessel was able to rectify prior to completion of inspection. 46 CFR 170.270(c)

Not less than 2 of the lifebuoys are required to have self-igniting lights shall be provided with lifebuoy self-activating smoke signals. Both smoke signals were expired.

An engineers alarm shall be operated from the engine control room and shall be clearly audible in the engineers' accommodations. Audible alarm had been unplugged.

Valves for drawing fuel or draining water from fuel shall be fitted with a cap or plug to prevent leakage. All main engine fuel filters did not have plugs or drains.

Each fall used in a launching appliance must be replaced every 5 years with end-for-ending them at 30 months, or replacing them every 4 years. Falls for the lifeboats and rescue boat have not been replaced since 12SEPT2016 with no evidence of end-for-ending.

Each Lifesaving appliance must be in working order and ready for immediate use. Starboard lifeboat must be made operational with engine running and hatches must be arranged to be able to remain in the open position.

Electric installations must be inspected to determine electrical condition and performance. Observed multiple lights out throughout the vessel.

Periodic Safety Tests must be conducted at periodic intervals to confirm that vital systems and safety features to continue to operate in a safe and reliable manner. Periodic safety tests have not been conducted since 2019. Additionally the annual testing binder does not include all of the automation tests that need to be tested included in the periodic safety test procedure binder.

Engines installed on a ship constructed on or after 01 JAN 2020 and prior to 01 Jan 2011 shall be tier 1 engines and prohibited from emitting nitrogen oxides within the limits in Marpol V1/13.1. Vessel could not find EIAPP cert and associated technical files for main engines that prove engines to be tier 1.

Vessels required to have a final emergency power source must be arranged so that each load under 112.15-1 can be energized including lighting for passageways, damage control lockers, and emergency equipment lockers. Found one e-light out on 0-1 deck, one out on 0-2 deck, and found fire locker on main deck not lit under emergency power.

Each lifesaving appliance must be in working order and ready for immediate use. The hydraulic fluid line for the rescue boat davit is leaking hydraulic fluid.

Each vessel must have posters or signs displayed in the vicinity of each survival craft for instructions on launching. Observed starboard lifeboat launching instructions were missing.

Each lifebuoy must be marked in block capitol letters with the name of the vessel and the name of the port required to be marked on the stern of the vessel under 46 CFR 67.123. Observed multiple lifebuoy markings that require new stenciling.

Non metallic expansion joint must be replaced 10 years after it is placed into service if located in a position required by 46 CFR 61.15-12. Two Saltwater cooling nonmetallic expansion joints on aft hydraulic system had no evidence of being replaced

Settings of safety relief valves on all pressure vessels must be checked at each inspection for certification. Vessel did not have evidence of the pressure vessel safety relief valves being checked within the last 5 years.

The safety of the vessel with automated vital systems shall be equal to that of the vessel with the vital systems under direct manual operator supervision. All automatically or remotely controlled or monitored vital systems must be subjected to test and inspections to evaluate the operation and reliability of alarms, controls, safety features, and interlocks. The following automation alarms and remote controls were found to be inoperable/faulty, and it is suspected there is more; low jacket water level sensor for Main engine 3 and port inboard generator; high temp alarm for both port and starboard main engine reduction gears; starboard smoke detector in main engine control room; aft bilge alarm; low start air pressure alarm for port inboard generator; multiple tank level sensors for fuel, ballast, and sewage; multiple remote valve controls over multiple systems; engineer stateroom call; sensor for emergency generator to alarm for water in fuel; remote tachometers for main engine turbochargers.

Watertight doors and hatches shall be watertight. Make adjustments to the Steering Room and Engine Room emergency escape quick-acting watertight hatches and the lower accommodations emergency escape quick-acting watertight door.

Fire doors shall be of the self-closing type with a latch. Make adjustments to the doors in the main stairwell to fully close and latch properly.

The lifeboat shall be provided with an approved compass with the proper mounting arrangement. The compass currently installed is missing fluid and mounted backwards. Provide and install an approved compass in the port lifeboat.

The lifeboat must have a position indicating canopy lamp. The current lamp has a cracked lens. Replace canopy lamp.

The condition of the ship shall be maintained to ensure that the ship, in all respects, will remain fit to proceed to sea without danger to the ship or persons onboard. Wash doublers and conduct gauging of the forward starboard potable water tank floor to determine the amount of wastage present and provide repair proposal to the attending CG amrine inspector no later than 31 Jan 2020.

Before the vessel leaves port & at all times during the voyage, each lifesaving appliance must be in working order & ready for immediate use. Sheaves, pulleys, & control wires are in need of maintenance on both P & S lifeboats.

Each penetration of & each opening in a bulkhead in a machinery space must: have means to make it watertight. Engine room bulkhead penetrations cannot be made watertight due to mismatching fittings.

Annaul inspections must include the following: (1) each survival craft must be stripped, cleaned, & thoroughly inspected and repaired as needed, at least once a year & each fuel tank must be emptied, cleaned, & and refilled with fresh fuel. (2) each davit, winch, fall, & other launching appliance must be thoroughly inspected & repaired, as needed, once each year. Vessel could not provide evidnce of of annual inspection for lifeboats or davits.

Provide approval documentation or approved rescue boat release mechanism IAW 46CFR199.160 (c) and 46CFR199.150(d) Prior to Sail

Provide Periodic Safety Testing Procedure verification prior to reduction of manning for minimally attended machinery space operations IAW 46CFR61.40-6(b) by 01AUG2017 and Prior to reduction of manning for minimally attended machinery space operations.

Provide distribution panel blanks to bow thruster panel, P-403, LP-204, and LDP-202 IAW 46CFR111.40-1 by 28APR17

Provide distribution panel latches to doors to LP-204 and LDP-202 IAW 46CFR111.40-1 by 28APR17.

Provide adequate emergency lighting near stbd generator IAW 46CFR112.01-10 by 28APR17.

Provide personal protection to stern thruster motor servo IAW 46CFR127.330 by 28APR17.

Provide secure junction box cover to stbd steering pump motor.

Found two cracks in the plate of the Dirty Oil Tank overhead/Engine Room bilge deck approx 1 foot aft of frame 47 or 10 feet of the entry hatch. Make repairs to the satisfaction of the Coast Guard.

Conduct operational test under load (46 CFR 199.190(5)) and update required equipment with corrosion resistant material, in the presence of a Coast Guard Marine Inspector.

Prove proper operation of the #2 MDE Jacket Water High Temperature Alarm. (46 CFR 62.25)

The WQSB procedures for M.O.B. under the [name withheld] duties, states to secure ventilation electrical power to space. Main Engine Room. Verify this procedure is correct & provide corrections to the Coast Guard. (46 CFR 131.350(c)(2))
