IAW 46CFR140.900, “Each towing vessel must comply with the requirements of part 4 of this chapter for reporting marine casualties and retaining voyage records.” Vessel was discovered on dry-dock making repairs where a hole was in the hull, due to a grounding incident. No notification was made to the Coast Guard or the TPO. The master did state he made notification to the Company, as per their ISM. The company shall conduct an ISM/DOC external management audit, in the presence of a Coast Guard Third Party Oversight Coordinator.

A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. MI observed doubler patch being installed over a hole on the bottom plate of the vessel which was caused by running aground. The Coast Guard or Third Party Organization were not notified and the repair was not authorized by the Coast Guard or TPO. The vessel representative must notify the TPO and objective evidence must be submitted to our office that authorized repairs were completed to the satisfaction of the TPO.

The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate.
