On each rescue boat type II retro reflective material must be placed on the boat to meet that arrangement requirements in IMO resolution A.58(16). Retro tape on the boats was delamintating and damaged. Replaced with approved tape and provide photos to the Coast Guard. 46 CFR 199.176

Fire pumps must be fitted with a pressure gauge on the discharge side of the pump. The discharge gauge was not reading pressure. Repair gauge and provide photos to the Coast Guard. 46 CFR 95.10-5

All marine engineering details installed on the vessel shall be IAW the provisions of subchapter F. The applicable standards established by ABS Rules (incorporated by reference, see § 58.03-1), may be used as the standard for auxiliary machinery. The turbochargers covered in this section are to be designed, constructed, operated, and maintained to: prevent the occurrence of fire and explosion. #1 Generator had an oil leak at the turbo charger. Repair leak and provie documentation to the Coast Guard. 46 CFR 90.20-1 ABS 4.2.2.1.2.1

Instead of the 2.5" hose specified by table 95.10-5(a) of this part, on vessels more than 1,500 gross tons the exterior hydrants may substitute two 1.5" outlets , each with a 1.5" hose, supplied through a wye connection. All exterior hydrants were fitted with a single 1.5" hose with no wye connection. 46 CFR 95.10-10

Radio lifesaving appliance installations and arrangements must meet the requirements of 47 CFR part 80. Both bridge to bridge radiotelephony and safety telephony certificates expired on 12JUL202 46 CFR 199.60(a) 16C

Fire detection, manual alarm and supervised patrol systems in special cases must meet the applicable requirement of 46 CFR, part 76 of subchapter H of this chapter. (continues on page 2) Fire detection audible and visual alarms did not work in engine control room or engine room space when fire alarm was activated. 46 CFR 95.05-1 16C

All marine engineering details such as piping valves, fitting, boilers, pressure vessels, etc., and their appurtenances installed on the vessel shall be designed, constructed and installed in accordance with provisions of subchapter F of this chapter. #1 Service pump had a hole in the 8” piping causing sea water to spray out. Permanent repair shall be enacted to the satisfaction of the marine inspector prior operation of equipment. 46 CFR 90.20-1 705C

The boundary bulkheads and decks separating the accommodations and control stations from cargo and machinery spaces galleys, main pantries and storerooms, other than small lockers, shall be of “A” class construction. ECR to Engine room and E-gen to ECR fire doors did not completely close during self-closing operations. 46 CFR 92.07-10 16C

Lifejackets must have the following attachments and fittings: (i) Each lifejacket must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the lifejacket. (ii) Each lifejacket must have a whistle firmly secured by a cord to the lifejacket. 02 lifejackets didn't have approved working lights while 01 lifejackets was completely missing a light. 46 CFR 199.70(b)(4) 16C

Each cable installation must meet— (1) Sections 6, of IEEE 45.8–2016 (incorporated by reference; see § 110.10– 1 of this subchapter); or (2) Cables manufactured to IEC 60092–353:2016 must be installed in accordance with IEC 60092–352:2005 (both incorporated by reference; see § 110.10–1 of this subchapter), including clause 8. The Roxtec multiple cable watertight transit penetration on the port aft bulkhead was improperly installed; thus, compromising the watertight and fire boundary integrity of the space. 46 CFR 111.60-5(a) 16C

Each vessel must have a line-throwing appliance; vessel did not have a line throwing appliance.

Bridge-to-Bridge Act applies to every dredge in or near a channel or fairway, in operations likely to restrict or affect navigation of other vessels; vessel did not have an FCC issued Bridge-to-Bridge certificate. 47 CFR 80.1001(d)

Each fire hydrant must have a spanner wrench; hose station #4 did not have a spanner wrench.

Each vessel must have posters or signs displayed in the vicinity of each survival craft's launching controls that illustrate the procedures for operating the launching device; rescue boat davit did not have an instruction placard for how to operate.

Illustrations and instructions showing method of donning lifejackets must be conspicuously displayed at each muster station; vessel was missing required placard showing how to don lifejackets.

Vessel shall minimize fire hazards insofar as is reasonable and practicable; missing plugs for fuel filters in e-gen room.

Suitable hand covers shall be installed in way of all exposed and dangerous places; duct smoke detector and coinciding wiring cover in mech room 106 was missing.

Each lifesaving appliance must be in working order and ready for immediate use; radar reflector on rescue boat was broken off.

Rescue boat must be marked with name of the vessel on each side of the bow and the number of persons for which the boat is equipped; rescue boat was missing these required markings.

A load line certificate shall certify to the correctness of the load line marks assigned to the vessel and that the vessel is in compliance with the applicable requirements. MI's unable to determine whether the load line issued to the vessel is for manned or unmanned operations. Provide evidence of a manned load line within 30 days or vessel will be restricted to operating within the boundary line.

The test in paragraph (a)(1) of this section must be carried out whenever a wire fall for a launching appliance is replaced or turned end-for-end. The proper condition and operation of the survival craft and rescue boat launching appliances at loads ranging from light load to 10 percent overload. Falls were replaced, a load test must be performed and witnessed by a CG Marine Inspector.

When the vessel is underway, the EPIRB must be stowed in its float-free bracket with the controls set for automatic activation and be mounted in a manner so that it will float free if the vessel sinks. The hydrostatic release is expired and must be replaced.

Paint lockers must have one 40-B portable extinguisher in the vicinity. Provide a 40-B in the vicinity of paint lockers.

Complete but simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room. On systems in which the CO2 cylinders are not within the protected space, these instructions must also include a schematic diagram of the system and instructions detailing alternate methods of discharging the system should the manual release or stop valve controls fail to operate. Instructions for CO2 System operation are missing from remote control station on port and starboard side of the vessel. Provide instructions at remote stations as required.

No person may operate an Ocean going ship unless it is fitted with approved 15ppm oil filtering equipment. Vessel removed thier oil filtering equipment. Vessel removed their oil filtering equipment. Vessel must have an approved equivalency or exemption as listed in 33 CFR 155.120/130 by next dry dock or OWS will need to be reinstalled.

A survey shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or whenever any important repairs or renewals are made. Vessel sustained a fire in the ER at engine to pump clutch assembly. Perform complete damage survey and provide repair proposal to CG Marine Inspector for approval prior to dredging operations. Upon approval, repair all damaged systems/materials to the satisfaction of the attending Marine Inspector. - 46 CFR 91.30 - 16c

Subject vessel is Certificated under the regulatory application of Subchapter I. 46 CFR 90.25-1(a) of Subchapter I requires that all electrical engineering details and installation be designed and installed in accordance with Subchapter J (Electrical Engineering). Vessel shall be equipped with two sources of electrical power. Three of the vessel's three ship service generators were removed from service due to mechanical failures in the prime mover. Repair or replace at least two of the damaged ship service generators to the satisfaction of the attending Marine Inspector. The installation of temporary sources of power shall be to the satisfaction of the attending Marine Inspector. Any testing the Marine Inspector deems necessary to approve temporary measures shall be conducted to his or her satisfaction. 46 CFR 111.10-3

Provide sufficient means to monitor alarms and running status of both temporary sources of power.

When acknowledging alarm on fire detection panel, a trouble light is indicated due to a blown fuse. Crew replaced fuse multiple times with same result. Provide technician report of repairs.

Galley rolling steal doors have no servicing record. Provide records to CG Marine Inspector.

Multiple self-closing fire doors were found out of adjustment, and unable to close. Re-adjust doors and retest for proper closure.

Exhaust lagging on #1 Generator was found excessively worn. Replace lagging and provide proof of repairs.

Vessel’s MSD (sewage tank) was found leaking. Identify leak location and perform repairs.

All three air receiver gauges installed max out at MAWP of air system. Gauges are required to be 1.2 to 1.5 times the designed pressure of the system.

Bulkhead penetration for battery maintenance station power cord was found without watertight conduit. Install approved watertight bulkhead penetration conduit.

1-2 ventilation air damper indicating light was found extinguished. Prove proper function of light.

Fire extinguisher located at sewage space access was found empty and bracket severely wasted. Replace fire extinguisher and bracket.

Multiple fuel/lube oil/hydraulic hoses on prome movers for both dredge pumps and 03 SSDG do not meet SAE J-1942 (cotton jacketed). Replace or install fire sleeves to meet SAE J-1842.

Multiple electrical panels are not marked with equipment serviced or breaker size. Mark all electrical panels to accurately represent service.

MSD is leaking from top access panels. Odor of chemicals and contents is present in space and catch basin is full of effluent. Repair.

Fire stations must be marked. Fire station #1 is not marked. Mark.

Each transfer pipe system must not leak under static liquid pressure at least 1.5 times MAWP. Transfer lines must be tested annually (or as part of biennial/ mid-period insp). Bunker lines have not been tested. Test bunker lines to 1.5 times MAWP.

The crew shall be protected from electrical hazards. Properly secure the battery selector switch for EDG.

Diesel fuel piping shall comply with 46CFR56.50-75. Make permanent repairs to soft patch on day tank overflow pipe.

A vessel o/o req'd to have a Vessel Response Plan shall conduct the minimum req'd exercises. VSL is not conducting quarterly QI notification drill or any exercises required by 33 CFR 155.1060. Conduct all req'd drills and exercises.

The transfer to the emergency power source must be automatic and accomplished in no more than 45 seconds after failure of normal source of power. The EDG failed to assume the load automatically. Affect repairs prior to departure to satisfaction of attending marine inspector.

Liferafts were in various states of improper installation. Painters were routed behind packing straps, painters were not connected to the weak link of the hydrostatic release units, and hydrostatic release units were secured to the vessel with nylon rope. Correct installation to the satisfaction of the attending marine inspector.

All Co2 Valves and controls shall be suitably protected. Port and Starboard Co2 pilot bottle valves are seized due to corrosion. Co2 unable to be activated as indicated on the Fire Equipment Plan. Provide report reflecting servicing and inspection of pilot bottle valves, attesting to their suitability for service as designed.

The Co2 cylinder storage space for the engine room Co2 system is not ventilated. Install ventilation iaw approved design plans. Demonstrate ventilation in the presence of a marine inspector.

There must be at least one shore connection to the fire main available to each side, in an accessible location. A fuel hose rack has been manufactured and attached to the starboard shore connection, as indicated on the fire equipment plan. Connection (ISC) is not readily accessible and is obscured. Remove fuel hose rack and ensure that the ISC connection remains unobstructed.

Approved OWS system includes 15 ppm automatic stop valve. Stop valve does not effectively stop the flow of oily water mixtures in excess of 15 ppm from being discharged overboard. Lock out/tag out, complete repairs, and conduct satisfactory operational test in the presence of a USCG marine inspector.

The previous name of the vessel, Weeks 315, is marked on the stern of the barge. The barge must be marked "C. R. McCaskill" on the stern to meet the requirements of 46CFR67.123.

Rescue boat has the incorrect outboard engine iaw manufacturer specification and USCG type approval. Verify that the outboard is approved for use or replace. Upon replacement conduct operational tests in the presence of a USCG marine inspector. Cites; 46 CFR 199.262(a), 46 CFR 160.156, and 46 CFR 199.190(i).

Unable to test

Deep fat fryer/griddle automatic fixed fire suppression system. Power has to be manually secured from panel located next to deep fat fryer. Power must be manually secured from a safe, remote location or automatically secured.

Install blank in distribution panel located in Galley.

Post required PFD donning Placard in crew space

Label ALL emergency lighting located in the Engine room/Mach Space.
