The vessel is built, equipped, and maintained to conform to the rules of a recognized classification society appropriate for the intended service and routes, but not classed. The vessel has damage to the #3 stbd fuel tank allowing water intrusion. Vessel will offload all fuel in #3 stbd. During transit, vessel shall increase rounds of vessel and soundings to maintain vessel's stability and proceed directly to dry dock for repairs. Following arrival to dry dock, prior to movement, determine extent and affect permanent repairs to the affected hull plating to the satisfaction of the TPO (TVIB). Provide objective evidence to the USCG.

The vessel is built, equipped, and maintained to conform to the rules of a recognized classification society appropriate for the intended service and routes, but not classed. The crew discovered two cracks in the hull of the forward hold following a high bilge alarm, affecting temporary repairs. The vessel is directed to proceed under an approved transit plan to Memphis to offload oily bilge water. The vessel is then directed to proceed to the designated offload facility for its tow of 16 barge, and then proceed to a designated shipyard facility to affect permanent repairs. Following arrival to this facility, prior to movement, determine extent and affect permanent repairs to the affected hull plating to the satisfaction of the TPO (TVIB). (46 CFR 144.200(a))

For a towing vessel utilizing the TSMS option, the COI is invalid upon the expiration or revocation of the owner or managing operator TSMS certificate of the ISM Code Certificate. 46 CFR 136.215(b) For those vessels selecting the TSMS option, the owner or managing operator of the towing vessel must provide to the OCMI objective evidence of compliance with the requirements in this subchapter prior to the issuance of an amended COI. 46 CFR 136.230(c) As of 30SEP24, vessel is no longer associated with the TPO stated on its COI. Prior to movement, vessel must complete a successful inspection by a Coast Guard Marine Inspector and provide a valid TSMS certificate to Sector New Orleans. Any movement from vessel's current location requires submittal and approval of a transit plan by Sector New Orleans OCMI.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. Vessel experienced loss of propulsion while underway due to low fuel level in day tank caused by faulty automatically controlled fuel transfer pump and day tank low fuel level alarm. Make effective repairs to satisfaction of vessel's TSMS Third Party Organization (TPO), iTow. OCMI requires TPO attendance to vessel to clear deficiency in accordance with 46 CFR 138.510. Provide approved transfer procedure in accordance with 33 CFR 155.720. Provide TSMS procedures for internal fuel transferring and for watch responsibilities relevant to fuel level monitoring.

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the associated fleet that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate.

Vessel's rudder angle indicator linkage vibrated loose from feedback unit rendering RAI inoperable in pilot house; repair system to operate as designed.

Make repairs to starboard rudder quadrant and prove proper operation.

Make repairs to port bow corner.

VESSEL'S THROWABLE LIFE SAVING RING WAS PAINTED YELLOW INSTEAD OF ORANGE AND DID NOT HAVE APPROVAL NUMBER ON IT.

GARBAGE DISCHARGE PROHIBITED PLACARD NOT POSTED.

VESSEL SHOWED EVIDENCE OF GARBAGE BEING THROWN OVERBOARD INTO RIVER.

VESSEL'S OIL TRANSFER PROCEDURES ARE INADEQUATE FOR TRANSFER OPERATIONS.
