Two means of escape must be provided. Emergency escape from engine room was inoperable. Make repairs, demonstrate ability to open excape scuttle. Engineer worked on it and fixed it immediatelly.

Propulsion Machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems must be maintained to ensure proper operation. While transiting the Illinois RIver, the vessels port engine unexpectedly became inoperable creating a loss of propulsion requiring an assit tug.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. The port main engine has a coolant leak. Vessel authorized to transit to Cairo, IL while conducting hourly rounds of port main engine coolant expansion tank and maintaining tank level fill indicator line. Notify the Coast Guard as soon as it is safe to do so if any of the following occur: -coolant leak rate increases, -coolant tank low level alarm sounds, -engine high temp alarm sounds, -or any other issues arise. Please work with your TPO (TVIB) to clear the deficiency IAW CG-CVC Policy Letter 17-10 while at next port of call Cairo, IL.

Vessel sustained damage to STBD engine resulting in shutting down the engine while pushed into RDB at LMR 767.6. Once the Third Party (TVIB) has cleared the deficiency they must provide documentation to this office within 72hrs.

“The functional requirements of a TSMS include: Procedures for reporting accidents & non-conformities.” At 11:17 on 11MAR2020, the vessel reported to the Sector Command Center a brief engine room fire from an exhaust vent. After reporting, the vessel continued underway, stating the vessel/company were following their TSMS Permit to Proceed Procedures by only “notifying” the OCMI, as outlined in section 11.32. This situation was deemed a marine casualty as outlined in 46CFR4.05-1, thus requiring both a notification and permission by the OCMI for any continued vessel movement. TSMS Section 11.32 does not incorporate the need for permission from the OCMI when a vessel sustains a marine casualty, in order to understand the situation, conduct a risk assessment, and provide any other necessary responses. Incorporate this difference into the company TSMS, and work with your TPO for the review and approved changes. 46 CFR 138.215(c)

“The functional requirements of a TSMS include: Procedures for reporting accidents & non-conformities.” Under TSMS 11.32 PTP, all subparts of section 4.0 procedures are extremely general and do not address any specific conditions of vessel non-compliance, as stated in 46 CFR 136.240(a)(2). No reference in this section addresses a fire, what equipment was affected, what equipment is to be inspected, if alternatives or equivalent levels are provided, risk assessments, roving fire watches, verification of fire detection systems, etc. Due to the insufficient existing procedures, for all future permit to proceed situations, the OCMI needs to be provided these procedures to ensure they properly address incurred non-compliance before the vessel continues underway. Expand these sections under 4.1 Procedures if the vessel plans to use the benefit of 46 CFR 136.240(a), and work with your TPO for the review and approved changes. 46 CFR 138.215(c)

Each towing vessel must be fitted with a general alarm that: Is activated at each operating station and can notify persons onboard in the event of an emergency; Is capable of notifying persons in any accommodation, work space (any area on the vessel where the crew may be present while on duty and performing their assigned tasks), and the engine room; Has installed, in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light that is identified with a sign that reads: "Attention General Alarm—When Alarm Sounds or Flashes Go to Your Station." When testing the general alarm, all spaces of the bottom engine room aft of the main engine space have flashing red lights, but no audio alarm can be heard when door hatches are closed. In the event lights are inoperable, crew would not be aware of a general alarm in the event of an emergency. Audio is required in all spaces per the cite, with additional lights in hard-to-hear spaces. When on the external decks of the bow and stern, and on top of the second deck, no general alarm audio can be heard, posing a hazard to the crew in the event of an emergency. Install general alarm devices in all of these referenced spaces before departure from Davant, LA. Item can be verified by the TPO, with a follow-up report submitted to [email withheld] within 24 hours. 46 CFR 143.235

Vessel experienced a fire in the engine room while underway. Vessel is to identify the cause of the fire, report the condition of the affected equipment, and ensure proper operation for safe navigation. Vessel is restricted prior to movement and make the vessel available for inspection. 46 CFR 143.204(a) Vessel allowed to move to Davant, LA on this day of 12MAR2020.

Vessel has experienced a fire with the boiler heating system. Vessel shall follow their Towing Safety Management System (TSMS). Vessel shall have Port Engineer/Tech Rep assess system damage and conduct corrective actions. Vessel shall keep system in the isolated/secured condition with all applicable electrical secured until system testing after repair. Vessel shall submit all findings and corrective action/tech reports to MSU CHICAGO via email [email withheld].
