There was multiple aerosol cans on engine room workbenches, cans of paint stored on engine room catwalk, portable gas buddy with gas stored out on deck, open buckets of grease and oil, etc.... Conduct a through walk-through of the entire vessel and properly stow hazardous items.

Both generators had cardboard shielding wire moused in place around the genset fan screen to minimize oil spray on the bulkhead, creating a fire hazard. Remove cardboard from the genset.

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must be maintained to ensure proper operation. The vessel’s starboard engine cooling water piping (forward and outboard) adjacent the water pump, was missing one of four bolts at a flange. A C-clamp was being utilized in place of the inadequate bolt. Remove C-clamp and install appropriate bolt for the flange.

Vessel experienced a loss of propulsion while outbound on the Houston Ship Channel after both main propulsion engines shut down. Once determination of cause, and satisfactory repairs are made to the satisfaction of the Third Party Organization (Sabine Surveyors), submit TPO report to Texas City OCMI. 46 CFR 143.205(C)
