Thermal Fluid heater must be operationally tested and checks of all safety and limit controls, combustion controls, programming controls, and safety relief valves must be conducted by the owner, Chief Engineer, or person in charge at the inspection for certification to determine satisfactory operation. The vessels Thermal Fluid Heater was unable to be tested due to low flash point cargo on board. System must be tested by marine inspector prior to loading any cargo which needs thermal fluid heater.

Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 millimeter (0.5 inch) diameter and is at least 25.4 millimeters (1 inch) long. It must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern. One of the vessels vapor headers was missing a vapor stud. Prior to loading/offloading any cargoes that require the use of the vapor recover system this stud shall be permanently affixed and verified by a Coast Guard Marine Inspector.

Vessel operators failed to provide a polymerizing cargo prohibition statement and/or the vapor headers were not open for inspection during the periodic exam. Provide the gas free opportunity to inspect the vapor header piping or provide the required prohibition letter to the attending no carriage of polymerizing cargoes in the past 12 months to the attending Coast Guard Marine Inspector.

Vessel sustained damage to its port side #3, #4, and stern voids as a result of an allision with the Mel Price Lock and Dam (St Louis area). Vessel is allowed to proceed to Baton Rouge, LA so that it can off load and prep for repair. Damage does not affect safe voyage of vsl w/cargo but shall be repaired before next cargo loading operation. Temp repair (red hand) is applied to stern fractures.
