Vessel reported port main diesel engine gear box failure due to failure of #6 bearing. Vessel shall make repairs to the satisfaction of the attending marine inspector.

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. Engine room ventilation louver on the port main engine exhaust stack, second deck, is missing 7 horizontal louver segments, preventing fie suppression abilities and allowing the uncontrolled release of carbon dioxide in the event the fixed fire suppression system is activated. 46 CFR 144.605

Carbon dioxide cylinders shall be located outside the spaces protected, and shall not be located in any space that might be cut off or made inaccessible in the event of a fire in any of the spaces protected. Between the rudder room which houses all the engine room carbon dioxide bottles and the separate generator space, the bulkhead door was tied back with a rope & unable to latch. Current condition not only poses a hazard to the integrity of the bottles in case of a fire, but reduces the ability/timeliness to extinguish a fire in the generator room due to the open door increasing the overall volume of air between the two spaces. Rope removed & handle properly repaired same-day. 46 CFR 76.15-20(a)
