Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure all bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Vessels had excessive fuel in the bilges. Clean bilges to the satisfaction of the attending CG inspector. 46 CFR 142.220 (a)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. The power cable for the davit to launch the vessel's rescue skiff over the side was improperly spliced into an extension cord which was also missing its ground plug, making the cord dangerous for use and rendering the skiff out of service. Properly wire device to make it safe for operation by the crew. Additionally, the master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. The rescue skiff was identified as part of man overboard procedures and due to the need to make electrical repairs, Coast Guard inspectors were unable to test the launching and operation of the skiff. Demonstrate proper launching, operation, and retrieval of the rescue skiff. 46 CFR 143.205(a), 46 CFR 141.230(a)

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. Vessel did not have adequate emergency lighting in the engineering spaces. Specifically, install additional lighting in propulsion shaft spaces, main engine space, and forward engineering space containing pressure vessels. 46 CFR 143.410 (b)

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. Unable to test all required engeenering alarms due to electrical panel being worked on by technincan. Additionally, general alarm did not flash required red light in engine room due to same panel being worked on. 46 CFR 143.230 (a), 46 CFR 143.235 (b)(3)

The ventilation attic fans in the engine room were inoperable. Vessel must demonstrate proper operation of engine room ventilation along with proper shutdown of ventilation system in case of fire. 46 CFR 143.205(a), 46 CFR 144.605

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Ceiling was removed in galley with exposed/uncapped wiring. Wiring in the pantry was hanging down with exposed, possible live end, at eye level. 46 CFR 143.400 (a)

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. Observed watertight door leading to shaft tunnel missing 04 securing brackets and compromising water-tight integrity. Install securing brackets and ensure adequate watertight integrity. 46 CFR 140.610 (b)

(a) Immediately after the addressing of resultant safety concerns, the owner, agent, master, operator, or person in charge, shall notify the nearest Sector Office, Marine Inspection Office or Coast Guard Group Office whenever a vessel is involved in a marine casualty consisting in - (3) A loss of main propulsion, primary steering, or any associated component or control system that reduces the maneuverability of the vessel; At 0030 near MM 132 vessel lost power on STBD generator and switched to port generator. Port generator ran for a few minutes then also shut down. Generators were back on-line at approx 0100. Failed to notify the Coast Guard for 8 hours and continued to conduct operations adding 7 more barges to the tow and proceeding northbound. Vessel shall provide corrective action report to USCG within 14 Days.
