A vessel must be operated in accordance with applicable laws and regulations and in such a matter as to afford protection against hazards to life, property, and the environment. During routine operations, the vessel suffered a galley fire that resulted in potentially severe damage to the windows, internal structures, electrical wiring, and galley systems. This vessel is permitted to transit upriver to Cairo and the Paducah, KY upon approval of, and IAW its transit plan. Upon arrival at its designated shipyard facility, prior to movement, determine the extent of the damage to the vessel and affect permanent repairs to the satisfaction of the TPO (Sabine Surveyors). Following completion of repairs, provide the associated survey report and closeout letter to the nearest OCMI and the issuing OCMI.

Machinery systems must be designed and maintained to provide safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Tiller yoke assembly serving the starboard steering rudder is damaged. Complete permanent repairs to the satisfaction of TPO.

All vessel equipment must be used in accordance with the manufacturer's recommended practice and in a manner that minimizes risk of injury or death. Vessel's Marine Sanitation Device (MSD) vent pipe was found disconnected in the forward hold resulting in flammable and toxic sewage gases to be released into the workspace. Make repairs to the satisfaction of the TPO.

Decks and bulkheads designed to be watertight or weathertight must be maintained in that condition. Vessel has multiple open penetrations in watertight bulkheads below the main deck. Make repairs to the following penetrations to the satisfaction of the TPO. - Wire penetration in watertight bulkhead separating forward hold and machinery space. - Seven open penetrations in the watertight bulkhead separating the engine room from shaft alley. Penetrations include wire penetrations, current pipe penetrations, and penetrations where pipes have been removed. - Two old pipe pipe penetrations in the watertight bulkhead separating shaft alley from the steering void. Conduct thorough inspection of each watertight bulkhead to ensure there are not anymore open penetrations.

Vessel experienced a shutdown of the port generator while underway at mile 535 Ohio River and moored to troubleshoot. It was determined that the generator throttle control actuator will need to be replaced. Vessel required to conduct repairs to the satisfaction of the TPO, TVIB and contact Coast Guard duty marine inspector at [phone withheld], when repairs are completed and before placing vessel back into service.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessels steering rudder ram failed due to limiter switch failing. Repair/replace as needed to bring steering gear back to full operational capabilities.

Vessel sustained damage to flanking rudder jockey bar and steering pumps due to debris jamming in rudders.

Vessels port winch failed causing the wire to release allowing the starboard barge string to collide with the Louisville River Front Park sea wall. The collision resulted in the 12 foot inset on the lead barge CH 9846 with a 12 inch fracture 10 inches above the waterline in the bow rake. The master of each vessel pushing ahead shall ensure that the gear used is appropriate for the arrangement of the tow, frequently inspected and remain serviceable. Vessel shall not use the port tow winch until the unit is repaired to the satisfaction of the attending Coast Marine inspector or replaced. If alternative methods are desired, submit a written proposal to the satisfaction of the attending Coast Guard Marine inspector. Maintain a 1 hour rotation watch on the damaged hopper barge CH 9846 to insure there is no flooding and have water discharge pumps in immediate
