Portable fire extinguishers must be maintained in accordance with NFPA 10. This requires the extinguisher to be installed securely on a hanger intended for the extinguisher. One aft B40 fire extinguisher was found not in the proper mounting bracket. Extinguisher must be placed in appropriate size hanger per manufacturer specs.

Engine Fire: Turbo charger failed one week prior; oil soaked lagging was re-installed - NOT replaced. Oil soaked lagging caught on fire. Found other areas on the engine that required lagging. Also found exhaust leak. Vessel made note of possible damaged propeller blades. Vessel made arrangements to make repairs at Brady’s Shipyard.
