Electric installations on vessels must ensure maintanence of systems integrity through compliance w/the applicable system requirements (IEEE, NEC, IEC, etc..) to which the system is approved. Marine Inspectors identified the starboard #3 alarm installation corroded and broken from the alarm sensor. Install/replace new cable connection to the #3 alarm censor on the cargo tank.

Gears, couplings, flywheels and all machinery capable of injuring personell shall be provided with adequate covers or guards. Inspectors found one guard missing hardware and was loose. Another belt was missing a guard entirely. Install machinery guard to belt on the starboard side of the machinery and install hardward to tighten loose guard.

All cargo tank hatches must be clearly marked in not less than 3 inches lettering "DANGER-KEEP OUT" and all hatches must be closed and dogged down. Marine Inspectors found several cargo hatches not labled "DANGER-KEEP OUT". Lable all cargo hatches large enough for human access on all the cargo hatches.

All tank vessels shall conform to the requirements in this subpart in construction of hulls. 46CFR32.60-1(a) Vsl was reported to be aground due to hurricane IDA. Rep and T&T Salvage company walked the vessel with reported damage of "Port side inset". Vsl shall provide salvage plan to the satisfaction of the CG prior to conducting salvage operations. After salvage is complete and vsl is re-floated, a one time transit is authorized to repair facility.
