Any tank vessel which is equipped with an internal combustion engine driven cargo pump on the weather deck shall be provided with a minimum of one remote manual shutdown station, conspicuously marked, and located at the midpoint of such vessel, or 100 feet from the engine, whichever is the more practical. The remote quick acting manual shutdown shall be installed on the engine so as to provide a quick and effective means of stopping the engine (such as by cutting off the intake air). Prime mover for cargo pump was not connected to the remote shutdown. 46 CFR 32.50 35(a)

Piping system components shall be suitable for use with the cargoes for which the barge is certificated, and shall be of materials listed in Subchapter F of this chapter, or such other material as the Commandant may specifically approve. All piping materials shall be tested in accordance with the requirements of Subchapter F of this chapter. The valve seat material, packing, gaskets, and all other material which comes into contact with the cargo shall be resistant to the chemical action of the cargoes for which the barge is certificated. Vessel reported a cracked weld where the cargo piping leading to the pump in the #3 starboard cargo tank meets the deck, which led to a small amount of asphalt leaking on deck. Vessel is authorized a 1 time discharge of cargo, but must complete repairs to the satisfaction of the Coast Guard prior to next cargo operations.

Vessel has run a ground on or near forward portside #1 wing tank. Vessel must proceed with plans to secure and free, by means of lightering if necessary in order to proceed to safe facility to unload cargo and conduct survey for damage and make repairs. Vessel is not authorized to conduct any further cargo operations or transits until damage survey and repairs are conducted to the satisfaction of Marine Inspectors/OCMI. Vessel shall keep US Coast Guard, MSU Chicago informed of all evolutions concerning damage survey, repairs, and operations to free from grounding.
