A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. Vsl hit a submerged object causing water ingress into the Port side #3 fuel tank. Conduct repairs to the satisfaction of the TPO and Provide TPO report attesting to a satisfactory Internal structural, and dry dock survey including tests of all repairs. Provide copy of the TPO report to the CG.

Resolution: 14'6"x2'x1/2" of bilge knuckle and a 3'x3'x1/2" bottom plate insert in the #3 port fuel tank was Cropped/renewed . A satisfactory air test of all repairs was conducted.

Each towing vessel must have a means to monitor and control the amount of thrust, rudder angle and (if applicable) direction of thrust at each operating station. Vessel lost primary steering. The KEVIN CONWAY must follow their TSMS and proceed to Mississippi Marine in Greenville, MS with an assist vessel for permanent repairs. All repairs and tests must be to the satisfaction of the TPO (TVIB).

To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover.(d) The valve must be operable from a safe place outside the space where the valve is installed. During test of the STBD remote fuel shutoff the valve did not close. Repair or replace the affected remote fuel shutoff valve/components and to the satisfaction of the TPO. Provide TPO report to the Marine Inspector. 46CFR143.260(d)

The cognizant OCMI may require additional exams of the vsl whenever he or she discovers or suspects damage or deterioration to hull plating or structural members that may affect the seaworthiness or fitness for the route or service of a vsl. These exams may include a drydock exam. Approx. 01 foot of water was discovered in the stern void. Crewmember stated there was a small unknown leak requiring approx. 01 feet of water in the void to be pumped out every 28 days. Provide Corrective Action Plan identifying the source of the water & repairs/actions necessary to maintain the vsl's seaworthiness as required and to the satisfaction of TPO and the CG. 46CFR137.300(b)

Vessel reported that back up generator is inoperable and needs replaced. Vessel has been granted permission to transit to MM 393 to make repairs. Vessel shall make repairs to the satisfaction of the their TPO prior to movement from MM 393. 46 CFR 143.205(a)

Master of vessel contacted the Coast Guard and reported the port engine turbo oil lubricating pipe fractured causing lubricating oil to spray on engine. Engineer shut down engine requiring the vessel to push in at MM 516. It was latter determined that the oil pipe that fed the port engine turbo is only needed while the engine was off to lubricate seals while the engine cooled down. Machinery must be designed and maintained to provide safe operations of the towing vessel. Vessel shall conduct temporary repairs to the port engine turbo that will ensure the satisfactory operation of engine and safety of the vessel and crew. Vessel shall proceed to final destination for final repairs. To the satisfaction of the recognized organization.
