Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Port and starboard exhaust piping is holed on second deck of machinery space in vicinity of exhaust supports. Make repairs and conduct testing to satisfaction of TPO prior to movement.
Action required: 60 - Rectify deficiencies prior to movement

Resolution: Received TPO report attesting to crop and renewal as original of damaged port and starboard exhaust piping. ABS tested and found satisfactory.

The functional requirements of a TSMS include: Procedures for reporting accidents and non-conformities. Objective evidence discovered during the renewal COI inspection indicate that the main engine exhaust leaks in the engine room were not properly documented or reported in accordance with the vessel's approved TSMS. An internal audit identifying causal factors and corrective action is required within 90 days.
