Remove dead end wiring in wheelhouse associated with bridge radio IAW 46CFR143.230(a)

Replace/repair oil pressure sending unit (port) and the guage for oil pressure portside in the engine room IAW 46CFR143.230.

Replace temp sensor on port generator IAW 46CFR143.230.

Refasten loose generator head tank. The tank is loose and ready to collapse. Repair IAW 46CFR143.205(a)

Machinery must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. Propulsion machinery, including main engines and systems must be maintained to ensure proper operation. During a scheduled damage survey, M/I’s expanded the inspection and discovered both main diesel engines, have been accumulating excessive air into the cooling water system preventing them from operating properly as designed. The crew reported having to bleed both keel coolers, particularly the port every three hours to prevent an overheat condition. It was reported by crew this condition on several occasions, when not bleed in time, has led to the expansion tank relieving hot water from the overhead of the engine room. Which poses a serious personnel hazard beneath, when doing rounds or maintenance.

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following; (a) All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids. M/I's observed an excessive amount of unmaintained oil and fuel mixture in the vessels engine rooms bilges. The crew stated this conditions had been had been ongoing for about three weeks. The total amount of mixture, appeared to be approximately 400-500 gallons. This posed a serious personnel hazard and an undue fire risk on board.

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following; (a) All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids. M/I's observed multiple fuel oil leaks on the following machinery: the port and starboard main diesel engines supply and return lines, including the starboard filter housing. The starboard generator had soaked rags which presented a fire hazard, the source appeared to be the high pressure injection pumps and lines.

Nothing in this subpart limits the marine inspector from conducting any test or inspections they deem necessary to be assured of the vessels sea worthiness or fitness for its routes and service. After removal of hatches for purpose of damage survey, it was discovered the forward space had approximately 4-8" of unmaintained water within the space. As well, the aft void space had approximately 2-3'. When asked about the amount of water in the space, the master stated he uses it as ballast. Given the void was only half full, M/I questions the stability arrangement and due to the lack of documentation attesting to the acceptability as utilized on board.

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. M/I noted the aft port engine room, water tight door was completely missing the gasket to make the door water tight as designed. Multiple hatches were observed wasted throughout the vessel. Additionally, the hatch forward leading into the storage room on lower fore deck, had hinges severally wasted preventing them from closing effectively and being watertight.

The company reported the vessel has experienced a leak in the keel cooler on the starboard main engine. During the initial haul out it was noted water was draining from the starboard side in the vicinity of the hull coolers. Determine the cause and extent of the repairs to be made proper in accordance with the provisions of 46 CFR 143.205. Provide a repair plan to be approved to the satisfaction of the attending Coast Guard.

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. During an expanded inspection, M/I's noted it took the crew 2-3 minutes of beating with metal sledge hammer on the forward fore peak hatch to gain entrance. Additionally, the same method was observed at the aft void space hatches to gain entrance. All three hatches were observed painted over and unmaintained.

Multiple cracks in way of port and stbd struts. Major wastage in both port and stbd struts. Hole in stbd stern tube. Crop and renew.

Both rudders have excessive play in the pintle and gudgeon bearings. Renew all associated bearings to acheive manufactures specifications.

A violation of 47 CFR 80.25 in that the UTV ROYAL ENGINEER was operating without a valid FCC ships license.

A violation of 33 CFR 164.72 (a)(4) in that the UTV ROYAL ENGINEER operating underway without a magnetic compass.

A violation of 46 CFR 25.25-5(d) in that the UTV ROYAL ENGINEER was operating with a substandard life ring buoy.
