A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford protection against hazards to life, property, and the environment. Vessel lost STBD main diesel engine for the second time after being cleared by USCG w/ 2 satisfactory tech reports. Prior to vessel movement, provide report explaining in detail root cause, action taken regarding prevention of future similar incidents, and prove proper operation of STBD main diesel engine to the satisfaction of the USCG Marine Inspector. If repairs are required and cannot be conducted at LMR MM 145.6 RDB, provide transit plan to the satisfaction of the USCG Marine Inspector to shipyard repair facility. Before vessel returns to service, provide proof of satisfactory sea trials conducted and proper operation of STBD MDE over speed shutdown and high temp shut down.
