GENERAL

All crew members must be provided with health and safety information and training that includes:(1) Content and procedures of the owner or managing operator's health and safety plan. Master was not completely familiar with the companies computer system for displaying information from the Health and Safety Plan. Please provide proof of documented training and familiarization to Master and crew. 46 CFR 140.515(a)(1)

Plastic pipe installations must be in accordance with IMO Resolution A.753(18) and IMO Resolution MSC.313(88) (both incorporated by reference, see § 56.01-2) and the following supplemental requirements. Vessel was observed to have installed on the MSD system, non marine approved household PVC piping in the machinery space. 46 CFR 56.60-25

Each towing vessel must perform the tests in Table 143.245(b). There is no record of any of the tests in the table. Complete alarm setpoints, record IAW 143.245(b) and provide objective evidence to a marine inspector. 46 CFR 143.245

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel experienced a loss of steering while underway due to fire in breaker panel. Vessel shall make repairs to the satisfaction of the attending marine inspector. 46 CFR 143.205(a)

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid. Port and starboard fuel tank man hole cover gaskets are worn, allowing the tanks to leak fuel into the engine room. 46 CFR 143.220(a)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Two fuel hoses on starboard propulsion engines are severely chaffed including chaffing of the inner core. Replace fuel hoses with hoses meeting SAE J1942 and hose fittings meeting SAE J1475. 46 CFR 143.205(a)

Piping and machinery components that exceed 220 degrees,including exhaust manifolds and turbo chargers must be insulated. Both generators are missing insulation on pipe elbow coming off of turbo charger. Make repairs within 7 days. 46 CFR 143.220(b)

Means must be provided for closing, in case of fire, each doorway, ventilator and annular space around funnels and other openings into machinery spaces. Vessel has no means to close ventilation openings on both stacks. Make repairs within 7 days.

Each towing vessel must have a fire-detection system installed to detect engine room fires. The owner or managing operator must ensure the following: the system is installed, tested and maintained IAW the manufacture's design manual. During inspection, vessel owner could not find the current manufacturer's design manual and was unaware of the recommended testing and maintenance requirements. Locate manual, review and draft a test and maintenance plan for the detection system.

Each towing vessel must have a fire-detection system installed to detect engine room fires. The owner or managing operator must ensure the following: The design of the system and its installation on the towing vessel is certified and inspected by a registered professional engineer with experience in fire-detection system design, by a technician with qualifications as a National Institute for Certification in Engineering Technologies (NICET) level IV fire alarm engineering technician, or by an authorized classification society with equivalent experience, to comply with paragraphs (a)(1) through (6) of this section. Vessel owner could not provide the documentation necessary to prove design and installation is certifed and inspected by a regsitered PE with experience in fire detection system design.

All portable fire extinguishers, semi-portable fire-extinguishing systems, fire-detection systems, and fixed fire-extinguishing systems, including ventilation, machinery shutdowns, and fixed fire-extinguishing system pressure-operated dampers on board the vessel, must be inspected or tested at least once every 12 months, as prescribed in paragraphs (a)(1) through (8) of this section. During inspection, vessel owner was unable to provide proof of annual testing of fire detection system.

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces; vessel does not have any closure devices for engine room to minimize fire damage.

The operator must ensure that the towing vessel is in compliance with Sub-chapter M at all times. Vessel has 2 holes in the forward rake bottom hull plating approximately two feet forward of aft bulkhead under the potable water tank. Code 705: Operator authorized temporary repair of doubler due to shipyard availability and shall complete permanent repair as soon as practical but no later than 15 October 2021.

Each towing vessel must have a high bilge level alarm, that is visible and audible at the operating station, and must have a means to test actuation at the operating station; marine inpsector found vessel had no high bilge level alarms in any required space.
